[03:27] Who would be asking you that question?
[03:29] And why do you think they'd be asking it?
[03:31] I think everyone can agree that sex is something we enjoy.
[03:34] It's something that's important that we want to have, right?
[03:36] And there is um a desire to have good sex or feel like maybe you're missing out on something that could be even better.
[03:44] So, how can you have the best sex possible?
[03:46] Because you hear all these people talking about amazing sex in the media.
[03:48] You might see it on pornography.
[03:50] You might feel like this is something amazing.
[03:52] Why am I not having that?
[03:54] I want that.
[03:56] Right.
[03:57] So, on that point, you said it's obvious that it's important.
[03:59] I think it appears to be increasingly not obvious that it's important because when you look at some
[04:06] important because when you look at some of these stats and I'll throw this graph up on screen
[04:10] and it shows that people are becoming more sexless especially young adults between 18 and 30.
[04:14] This graph is pretty stark.
[04:17] Mhm. Think about 50 years ago.
[04:19] There was no cell phones.
[04:21] There might be some TV programs but there was ads in between TV programs and there was only certain TV shows you could watch at a certain time that you enjoyed and then there was nothing to watch.
[04:28] There was no email.
[04:30] So, you went to work and you came home and there was really not a lot of communication between you and your co-workers or your job.
[04:34] They made dinner with their family.
[04:35] They hung out and then maybe they watched a program or two and then they lied down in bed.
[04:41] They didn't have a phone to scroll on.
[04:43] They didn't have anything to keep them up.
[04:45] And so then they were like next to their partner and maybe they're talking, maybe they're cuddling.
[04:50] And so there was more like opportunity and space for sex, right?
[04:52] You it was a thing that you did for enjoyment, for pleasure, for fun.
[04:56] Now we have all these distractions that keep our mind away from sex.
[04:59] In order to enjoy sex, you need to have space for it to be able to enjoy your partner and
[05:07] to be able to enjoy your partner and feel like you want that, right?
[05:09] And now we're sort of there's not as much room for it.
[05:11] That's one.
[05:14] Two is younger people, how are they dating, right?
[05:16] How are they meeting each other?
[05:17] They're meeting each other through apps, through um DMs.
[05:20] And so they're not actually like really having these deep meaningful connections.
[05:25] there have there's a big hookup culture.
[05:27] There's a big like fling culture.
[05:29] And imagine first time sex for most people is not great.
[05:31] You don't know your partner.
[05:33] You don't know what they like.
[05:34] And you may not be able to express what you like.
[05:36] You're never taught how to talk about sex, right?
[05:38] So you don't know how to say like, "Hey, I like it like this."
[05:40] And you may be self-conscious cuz it's your first time.
[05:41] So you have like mediocre sex or bad sex.
[05:43] And they're like, "Well, I don't really want that."
[05:44] And there's just no like follow through, right?
[05:46] There's there's not a lot of um education on what sex should look like.
[05:48] like the only education people are getting are from often erotic films and so they're seeing sex that's not real that's curated and they're trying to emulate that and that's not pleasurable for most people and younger people these days are um
[06:08] and younger people these days are um actually having a rise in more what we call rough sex so choking has become very very common as common as like they call choking almost like vanilla sex so young people if you look at data like 60% of women and I think 20% of men have been choked during sex of that age group, like college age group.
[06:30] And of those people who get choked, 20% have been choked 25 times or more.
[06:35] Now, I think it's fine if you're into that, but I can't imagine that that many people are into choking.
[06:40] And there's data to support that.
[06:41] So, when you look at qualitative data, and this is by uh Debbie Herbick, she's a sex researcher, and she did qualitative research, and she asked people like, "What is it like?
[06:47] Do you enjoy being choked? Tell us about it.
[06:49] Right? Women were like, "Yeah, it's okay.
[06:51] Sometimes I'm scared because my partner is big and strong and I'm, you know, smaller and they have big hands.
[06:57] Sometimes it's fine and sometimes I don't really care for it, but like it's just a part of sex.
[07:01] It's just as normal as as kissing, for example.
[07:03] And so if you're having sex and you're doing things just because you think that's
[07:09] things just because you think that's what your partner wants or what it what your partner wants or what it should look like and you're not enjoying it, well, that's not going to be sex that you want to have.
[07:14] that point you said about the the lifestyles we lead and how that might be impacting our sex lives I thought was really interesting because we don't talk enough about this but when you look at some of the data on this demographic um you know people 30 and below around that age the extreme outliers are spending 8 to 10 hours on their phones on social media and on the internet and roughly about 15 to 20% of young people describe their usage as almost constant effectively scrolling during all waking hours while they're eating, while they're in the bathroom, and before they go to sleep.
[07:49] So, and I was wondering from a dopamine perspective if there's correlation between these like dopamogenic activities that are now like hijacking our lives, whether it's short form videos on social media, whether it's pornography or food, whether it's having an impact on what then happens in the bedroom and our performance in the bedroom.
[08:08] If you think about what you need to have
[08:10] If you think about what you need to have good sex is you need to be sort of in the mind space for sex.
[08:14] And if you're constantly like hijacked by all these other things, you're never like really getting in the mood, right?
[08:20] You're just like, "Oh, I'm going to be turned on when I see my partner and we're going to have sex and it's going to be over."
[08:25] And it's just really a mechanical thing at that point, right?
[08:27] You're not actually you're just trying to get an orgasm.
[08:30] You're not actually like spending the time to enjoy and to experience that fully because you're just so like you're just your brain is always doing something else, right?
[08:38] because people are constantly scrolling like every 60 seconds there's a new video, there's something else.
[08:45] And so it's really hard to focus.
[08:47] And so that can translate to the bedroom where you're like you're having sex, but your brain is somewhere else thinking about something else you saw or something you have to do or something you want to see or something you want to look up, whatever it is, but you're not really in there in the moment.
[08:59] You're just going through the motions.
[09:00] And so I think that's really where the challenge is is that people are becoming increasingly distractable.
[09:07] I find it really hard, I got to be honest, to have sex if I've had like a really really busy day or if I'm really
[09:11] really really busy day or if I'm really really thinking about something.
[09:13] I really thinking about something.
[09:14] I almost have to I have to like intentionally create quite a lot of space.

[09:17] In order to be able to be in the mood.

[09:22] It takes like time and energy.

[10:14] And for me as well, it's not just touch like that doesn't necessarily for me, it really is quite like a psychological thing.
[10:19] I was wondering if this is there's any data around this or I mean just even anecdotally like people get aroused in very very different ways, don't they?
[10:27] Yeah, absolutely. I mean some people um are very like visual so they you know they see their partner and they get aroused very quickly.
[10:36] Basically when you get aroused you need to be in a parasympathetic nervous system state.
[10:39] So in order to get an erection you need to be in the state which is like rest and digest.
[10:43] So if you're stressed if you're thinking about other things if you're essentially on the go you are not allowing your nervous system to calm down.
[10:51] And so for some people that's a whole bunch of different things.
[10:53] Some people can switch more easily into that state and some people need more of like uh you know to feel either mentally stimulated or they need to have some associations like it might be like they need a certain scent, they need to like relax their body, they need to like go take a bath, whatever it is, but some
[11:13] take a bath, whatever it is, but some people need different things and knowing people need different things and knowing what that is for your partner is super important, right?
[11:19] Because then you can incorporate that.
[11:21] Everything is scheduled in our lives, right?
[11:22] And then you're like, "Oh, but now sex is like the last thing on the schedule, right?"
[11:27] And like I don't even think about it.
[11:28] Like, "Oh, okay. Now, yeah, maybe let's have sex."
[11:29] But it's like, if you actually make time to be intimate, allow yourself to be in that brain space, be together, that's when it can actually happen.
[11:37] Especially when you've been in a long-term relationship, it doesn't come as easily like spontaneously, which we call spontaneous desire.
[11:43] It it comes more as a response to these other cues that allow you to feel desire and to feel aroused.
[11:50] H I also think I was thinking about a previous relationship I had where on the days where I'd been like working very hard and I was like tired or stressed or I've been traveling and I was jetlagged.
[12:03] I think there was also because I didn't see this person often there was also an expectation that when I did see them we were going to have sex.
[12:07] >> and um that was that was very hard because actually the expectation of it
[12:14] because actually the expectation of it stressed me out more.

[12:18] Yeah. And that can happen like if it becomes the elephant in the room it can become a little bit heavy which then is sort of counterproductive to performance.
[12:24] and this is you know sort of a variation on performance anxiety.
[12:26] So when you feel like you have to perform on demand and maybe you're worried it might not happen it creates this vicious loop right so you may maybe have trouble with an erection or maybe it's like I I'm expected to have sex but I really can't get there mentally.
[12:39] whatever it is. Now, you're thinking about that, right?
[12:41] And then you're with your partner.
[12:43] You're like, "Oh my god, am I going to have trouble?"
[12:45] You're not enjoying the pleasurable sensations or the visuals or like feeling each other.
[12:48] You are literally thinking in your own head about how you're going to respond.
[12:51] And then that anxiety makes it so that you can't get an erection or you can't be aroused.
[12:54] And so now you're like anxious and you're not focused.
[12:57] You're almost spectatoring.
[12:59] You're just watching yourself have sex.
[13:01] You're not actually like in the moment.
[13:02] So then, you know, you have a negative outcome because when you're stressed, your sympathetic nervous system is on, right?
[13:06] You can't really get an erection or you can't really get aroused.
[13:09] And so then you're
[13:15] really get aroused.
[13:17] And so then you're like, "Oh man, now I've let my partner down.
[13:19] Now I haven't performed," which I hate that word, but like performed the way I should or or I'm expected to.
[13:24] And now something's wrong with me.
[13:26] And now that just keeps going in a vicious cycle.
[13:28] cycle.
[13:29] Have you spoken to people that have experienced this?
[13:31] Absolutely.
[13:32] It's common.
[13:34] Very common. I tell people anytime you have problems in the bedroom, it stays with you.
[13:38] with you.
[13:39] So, how do you break the cycle?
[13:41] Yeah. So, I tell people when you're with your partner, take the pressure off um penetration.
[13:44] Just explore each other's body.
[13:47] Do what's called like sensate focus.
[13:48] Like explore the rest of your body.
[13:50] Figure out other arogynous zones, other things that can turn you guys both on that don't involve erections and penetration.
[13:55] And then once you realize you're focused on that, you're really like exploring, enjoying, playing, you're having a good time, and you're not thinking about your erection, now you'll notice, oh, the erection just comes, right?
[13:59] And then once you realize you're focused on that, you're really like exploring, enjoying, playing, you're having a good time, and you're not thinking about your erection, now you'll notice, oh, the erection just comes, right?
[14:00] And then once you get to that point, then you can start even touching genitals, but still hold off on penetration.
[14:02] And then after you've realized like the genitals are, you

[14:19] I'm not stressed.
[14:20] Then finally, you can then introduce penetrative sex again.
[14:23] So, it's just sort of like a gradiated sort.
[14:24] of slow advance into um you know, having sex again, but now kind of focusing on.
[14:31] being more present and mindful and enjoying those sensations.
[14:33] I think a lot of people will be able to do that, but there's also a big contingent of people that just avoid sex.
[14:37] Mhm.
[14:42] It's it's a sore subject in their relationship for whatever reason.
[14:44] Both partners don't know how to communicate.
[14:47] They haven't got the tools to talk about these kind of things openly and honestly.
[14:51] Do you see that a lot as well?
[14:53] Absolutely.
[14:53] I mean, what do you hear in this regard?
[14:55] So, I think it's really sad.
[14:58] I see people come in and they're like, I ask every patient like, "Are you are you having sex?
[15:00] Why not?" Right?
[15:02] Because sometimes they'll say no.
[15:03] And most doctors will be okay.
[15:05] But I always say, "Why not?" Right?
[15:06] Why are you not having sex?
[15:08] Is it because you're having an issue?
[15:09] Is it because you're having pain?
[15:11] What's going on?
[15:13] Right?
[15:13] And often times I'll hear from people that my partner is
[15:16] I'll hear from people that my partner is just not into it and I just sort of gave up and I'm just we just don't have sex anymore.
[15:23] You know, for me that's a red flag because sex is a huge important part of our lives.
[15:29] It is a way we connect with another human being.
[15:34] It's also telling that things are working really well.
[15:38] So, when you have good sexual function, meaning you get an erection well or you get aroused well and you have a good orgasm and everything feels good, that tells me that, hey, you've got great blood flow to your genitals, your nerves are working great, your hormones are sending signals, like all these things are good, right?
[15:53] But also, sex is more than just the act of sex.
[15:58] It also helps you live longer.
[16:01] So, there's been a few studies looking at sex and longevity.
[16:05] And when you look at people who have sex once a week compared to people who have sex once a year, the difference in all cause mortality is 49%.

[16:27] There was actually interesting study in 1997, I think it was, where they looked at the number of orgasms men had, and they found that men who had for every hundred orgasms men had, they lived like 13% longer.
[16:43] They had a 13% increase in life expectancy.
[16:45] And so, it was really interesting to me just just showing you that like this is not just an act of pleasure and fun.
[16:52] It is obviously, but it's much more than that because people who are having sex clearly have better health and there this connection with people.
[16:58] I mean, loneliness is a big issue right now.
[17:02] The the WHO made loneliness like an epidemic.
[17:03] So, they've said that loneliness is as bad as having like 15 cigarettes.
[17:09] And so, sex is a way to feel connected to another human being.
[17:11] On that data, we're not saying that it's the sex itself that's causing people to live longer.
[17:16] We're I guess it's it's hard to establish causation in terms of
[17:18] hard to establish causation in terms of.
[17:21] Yeah. It's not necessarily sex, but they've looked at like they try to control for other things like age and coorbidities and all these studies and um it's also like sex is a cardiovascular workout, right?
[17:30] For many people is a cardiovascular workout.
[17:32] You are getting a phys physical physical activity with your partner.
[17:35] Um you are increasing your heart rate.
[17:37] You are doing these things that are also good for your body.
[17:41] And the fact that you're able to have sex, right, tells me a lot about it, right?
[17:45] You're you're able to hold a certain position.
[17:47] you're able to u maintain this level of activity without getting short of breath, right?
[17:51] Like these are things that having sex, you know, keeps you healthy to some degree.
[17:56] If I want to make sure that I have great sex, what are some of the foundational things that I need to be thinking of in terms of my lifestyle?
[18:04] Yeah. So, there's four main pillars of sexual health.
[18:06] I like to think of it like your sexual health is your house.
[18:08] And these pillars are the foundation.
[18:11] And if you don't have the foundation, it doesn't matter what else you do.
[18:13] Um, you can try to do everything else to patch up your house, but it's always going to break again because the foundation's not there.
[18:18] So, you've got fuel, and fuel is
[18:22] So, you've got fuel, and fuel is how you nourish your body.
[18:23] A lot of the data I'm going to talk about is about men because there's just a lot more data on men and sexual health, but that doesn't mean that the same things don't apply to women.
[18:32] There's just less less robust data on it.
[18:34] So, when you talk about fuel, the Mediterranean diet is the most studied diet.
[18:39] So, including things like healthy fats like avocados, leafy greens, nuts, and we're going to talk about nuts in a little more detail.
[18:47] These things are super helpful.
[18:49] And obviously having lean proteins, having an abundance of fruits, which we're going to talk about as well.
[18:54] There's a study called the Health Professionals Follow-up Study.
[18:57] They look at 20,000 men, and they saw that men who adhered to a Mediterranean diet had a 22% lower risk of erectile dysfunction.
[19:07] So what specific things in that diet, right?
[19:09] People always like, "What are the superfoods I need to have?"
[19:11] Almonds are great, but pistachios, they actually did a study on pistachios where they looked at 100 grams of pistachios.
[19:17] Guys ate 100 grams of pistachios every day and they saw a decrease in erectile dysfunction.
[19:23] So pistachio nuts will make my penis harder.
[19:23] harder.
[19:25] I mean, so I always say like I don't love to talk about superfoods because then people like, "Oh, I just got to eat pistachios and it's all good, right?"
[19:31] It's it's part of a whole diet, but certainly having nuts because they have great omega-3s, they have healthy fats.
[19:37] These are the reasons that they they really sort of improve diet.
[19:42] Fruit um anything that has sort of flavonoids, so like colorful fruit like blueberries, citrus fruits, um lycopine, which is red fruits, all of these things improve antioxidants and also have been shown to reduce the incidence of erectile dysfunction.
[19:55] So having stronger erections.
[19:57] specifically blueberries actually came out to have I think something around 20% also improvement in erectile function when you're eating blueberries regularly.
[20:06] So lots of I think things in the diet that can be helpful.
[20:09] Also fiber is one that we don't talk about enough.
[20:12] When you eat fiber in your gut it converts to short- chain fatty acids.
[20:15] These short- chain fatty acids then sort of have these endothelop protective mechanisms.
[20:20] They protect the blood vessels.
[20:22] They make them healthier.
[20:23] And so when your blood vessels are
[20:24] And so when your blood vessels are healthier, you get better metabolic
[20:25] healthier, you get better metabolic health. So you get less diabetes, less
[20:27] health. So you get less diabetes, less high blood pressure, less high
[20:28] high blood pressure, less high cholesterol. And these all of these
[20:30] cholesterol. And these all of these things together improve erectile
[20:32] things together improve erectile function.
[20:33] function. >> So I think making sure that you meet the

[20:38] criteria for fiber, which is 38 grams for men, 25 grams for women, um, is

[20:43] really really important. And then obviously managing your calories and a
[20:45] obviously managing your calories and a maintenance, right? because we don't
[20:46] maintenance, right? because we don't want to gain weight because excess
[20:48] want to gain weight because excess atapost tissue or excess fat also puts

[21:10] exercise. The one study that's quoted very often is 150 minutes of exercise of
[21:13] very often is 150 minutes of exercise of cardiovascular exercise moderate
[21:14] cardiovascular exercise moderate intensity. when you look at the
[21:16] intensity. when you look at the improvement in erectile function scores,
[21:19] improvement in erectile function scores, it is the same amount of improvement as
[21:21] it is the same amount of improvement as you would see when you take a medication
[21:23] you would see when you take a medication like Viagra. So, literally, if you do
[21:26] like Viagra. So, literally, if you do 150 minutes a week, you are getting the
[21:28] 150 minutes a week, you are getting the same improvement as you could get with a
[21:30] same improvement as you could get with a medication potentially. And so, I tell
[21:32] medication potentially. And so, I tell people like, look, if you don't want to
[21:34] people like, look, if you don't want to take a pill, this is a great way to
[21:36] take a pill, this is a great way to improve sexual function. Now, you might
[21:38] improve sexual function. Now, you might say, okay, well, I, you know, maybe
[21:40] say, okay, well, I, you know, maybe you're listening and you have heart
[21:42] you're listening and you have heart issues and you can't really do moderate
[21:43] issues and you can't really do moderate intensity exercise. They actually looked
[21:45] intensity exercise. They actually looked at that too. So there was a group where
[21:46] at that too. So there was a group where they looked at men who had heart disease
[21:49] they looked at men who had heart disease and they weren't really able to do
[21:51] and they weren't really able to do moderate intensity exercise. So they did
[21:52] moderate intensity exercise. So they did like a 5minute warm-up. They did 20
[21:54] like a 5minute warm-up. They did 20 minutes of walking and 5 minutes of a
[21:56] minutes of walking and 5 minutes of a cool down. And with this supervised
[21:58] cool down. And with this supervised protocol, they still improve erectile
[22:00] protocol, they still improve erectile function by 70%. So it's all relative to
[22:03] function by 70%. So it's all relative to where you're starting. But any sort of
[22:06] where you're starting. But any sort of cardiovascular exercise that's above
[22:08] cardiovascular exercise that's above what you are capable like what you're
[22:10] what you are capable like what you're doing now is going to improve sexual
[22:12] doing now is going to improve sexual function particularly erectile function
[22:14] function particularly erectile function cuz it's going to improve blood flow to
[22:15] cuz it's going to improve blood flow to the genitals going to improve blood flow
[22:17] the genitals going to improve blood flow to the penis. It's going to improve
[22:18] to the penis. It's going to improve blood flow to the clitoris for women. So
[22:20] blood flow to the clitoris for women. So that's where cardiovascular exercise is
[22:22] that's where cardiovascular exercise is so so important in terms of resistance
[22:24] so so important in terms of resistance exercise. I know these little weights
[22:26] exercise. I know these little weights are probably not sufficient for doing
[22:28] are probably not sufficient for doing much, especially for men, in terms of
[22:30] much, especially for men, in terms of improving um muscular health, but
[22:32] improving um muscular health, but obviously we're not going to bring like
[22:34] obviously we're not going to bring like really heavy weights to the table here.
[22:36] really heavy weights to the table here. So, one, we know that resistance
[22:38] So, one, we know that resistance training is significantly correlated
[22:40] training is significantly correlated with testosterone. So when you do heavy
[22:43] with testosterone. So when you do heavy resistance training of your large muscle
[22:45] resistance training of your large muscle groups, so like your lower extremities,
[22:47] groups, so like your lower extremities, your glutes, you're doing like Olympic
[22:49] your glutes, you're doing like Olympic deadlifts, squats, that sort of stuff,
[22:51] deadlifts, squats, that sort of stuff, you actually see improvements in
[22:53] you actually see improvements in testosterone. Now, it's not going to be
[22:55] testosterone. Now, it's not going to be like you do it once and you get this
[22:57] like you do it once and you get this sustained improvement, but continuous
[22:59] sustained improvement, but continuous sort of regular resistance exercise
[23:01] sort of regular resistance exercise improves testosterone.
[23:03] improves testosterone. There's also data that shows that when
[23:05] There's also data that shows that when men do resistance exercise to maintain
[23:07] men do resistance exercise to maintain muscle mass. So, we know that muscle
[23:09] muscle mass. So, we know that muscle mass decreases about 7% every decade of
[23:12] mass decreases about 7% every decade of life after around 40. When you maintain
[23:14] life after around 40. When you maintain it through resistance exercise, they're
[23:17] it through resistance exercise, they're three times less likely to have erectile
[23:19] three times less likely to have erectile dysfunction.

[24:07] and we're just doing less and less of it.

[24:43] And so, you can see this bowl of muscles here, right? Interesting. I've never

[25:02] this model. In this model, you can see they kind of show you the muscles here
[25:03] they kind of show you the muscles here on the side.

[25:21] your hips, your sacrum, your pubic symphysis. They attach to all these and
[25:24] symphysis. They attach to all these and they sort of just work in the background
[25:26] they sort of just work in the background for most people. How they affect your
[25:28] for most people. How they affect your sexual function is when you orgasm,
[25:31] sexual function is when you orgasm, these muscles contract and release at a
[25:35] these muscles contract and release at a at a rhythmic contraction of8 seconds.
[25:37] at a rhythmic contraction of8 seconds. And so you may feel that, right? There's
[25:39] And so you may feel that, right? There's like this pulsing feeling when you
[25:40] like this pulsing feeling when you orgasm. And that's these muscles sort of
[25:42] orgasm. And that's these muscles sort of doing that. And when men ejaculate, the
[25:45] doing that. And when men ejaculate, the pelvic floor muscles are contracting to
[25:47] pelvic floor muscles are contracting to help shoot the ejaculate out. They
[25:49] help shoot the ejaculate out. They squeeze when you need to keep things in.
[25:51] squeeze when you need to keep things in. So they'll keep urine in, they'll keep
[25:53] So they'll keep urine in, they'll keep um your stool in, and they relax when
[25:55] um your stool in, and they relax when you need to pee in, when you need to
[25:57] you need to pee in, when you need to defecate.
[25:58] defecate. >> So when you're trying to hold a wee,
[25:59] >> So when you're trying to hold a wee, you're like tightening your pelvic
[26:00] you're like tightening your pelvic floor.
[26:01] floor. >> Correct. Correct. But these are also
[26:03] >> Correct. Correct. But these are also responsive to stress. So, just like
[26:05] responsive to stress. So, just like people get TMJ where they get tense in
[26:08] people get TMJ where they get tense in their jaw cuz they are stressed and they
[26:10] their jaw cuz they are stressed and they like sleep at night and they clench up
[26:11] like sleep at night and they clench up their jaw. They don't really know
[26:12] their jaw. They don't really know they're doing it, the same thing can
[26:14] they're doing it, the same thing can happen. These muscles can get very tight
[26:16] happen. These muscles can get very tight or they can get misaligned. So, say you
[26:18] or they can get misaligned. So, say you have a hip injury or say you have a back
[26:20] have a hip injury or say you have a back injury, the muscles can compensate by
[26:22] injury, the muscles can compensate by tightening up. And so, a lot of people
[26:24] tightening up. And so, a lot of people unknowingly have tension in these
[26:26] unknowingly have tension in these muscles and it can present in a multiple
[26:29] muscles and it can present in a multiple different ways. It can present with back
[26:31] different ways. It can present with back pain. It can present with constipation.
[26:35] pain. It can present with constipation. It can present with urgency frequency
[26:37] It can present with urgency frequency because remember your bladder is sitting
[26:39] because remember your bladder is sitting right here on top of these muscles. So
[26:41] right here on top of these muscles. So when the muscles are tense, your bladder
[26:43] when the muscles are tense, your bladder is feeling like there's something
[26:46] is feeling like there's something something activating it. And so it's
[26:47] something activating it. And so it's like, oh man, I got to pee. That means I
[26:49] like, oh man, I got to pee. That means I have to pee. This tension is telling my
[26:50] have to pee. This tension is telling my bladder I have to pee. But it's really
[26:52] bladder I have to pee. But it's really that your bladder is not that full. It's
[26:53] that your bladder is not that full. It's that these muscles are telling you to do
[26:55] that these muscles are telling you to do that. You can also have trouble peeing
[26:57] that. You can also have trouble peeing because you can see that your urethra
[26:58] because you can see that your urethra goes through here. And if the muscles
[27:00] goes through here. And if the muscles are really tight, sometimes it can be
[27:01] are really tight, sometimes it can be difficult to urinate because it clenches
[27:03] difficult to urinate because it clenches off the pee. And then with sex, it can
[27:06] off the pee. And then with sex, it can cause pain. If they're really tight, it
[27:08] cause pain. If they're really tight, it can prevent blood from getting to the
[27:10] can prevent blood from getting to the genital organs. So for men, they can
[27:12] genital organs. So for men, they can have erectile dysfunction. For women,
[27:15] have erectile dysfunction. For women, they can have difficulty getting orgasms
[27:17] they can have difficulty getting orgasms or difficulty getting arousal because
[27:19] or difficulty getting arousal because they're not getting blood flow to the
[27:21] they're not getting blood flow to the clitoris. Sometimes they can also cause
[27:23] clitoris. Sometimes they can also cause premature ejaculation in men. And so
[27:25] premature ejaculation in men. And so these muscles are so important and all
[27:28] these muscles are so important and all we hear about is keigull and keigull are
[27:30] we hear about is keigull and keigull are exercises to strengthen these muscles.
[27:32] exercises to strengthen these muscles. But keigull are good when you have a
[27:34] But keigull are good when you have a normal pelvic floor. Meaning like
[27:36] normal pelvic floor. Meaning like there's no tension. It's completely
[27:38] there's no tension. It's completely normal. It's acting normal. You're not
[27:39] normal. It's acting normal. You're not having any symptoms at all. But if you
[27:41] having any symptoms at all. But if you have any of the symptoms I talked about,
[27:42] have any of the symptoms I talked about, doing keles might make it worse because
[27:44] doing keles might make it worse because you're now tightening muscles that are
[27:46] you're now tightening muscles that are already tight.
[27:47] already tight. >> We also talked about pelvic floor
[27:49] >> We also talked about pelvic floor relaxation. Doing exercises to
[27:51] relaxation. Doing exercises to specifically relax these muscles. So
[27:53] specifically relax these muscles. So that can be diaphragmatic breathing.
[27:56] that can be diaphragmatic breathing. That can be doing like a figure four
[27:58] That can be doing like a figure four stretch. That can be doing happy baby
[28:01] stretch. That can be doing happy baby pose which are yoga poses or child's
[28:03] pose which are yoga poses or child's pose. All these things can sort of
[28:05] pose. All these things can sort of stretch and lengthen these muscles so
[28:07] stretch and lengthen these muscles so they can learn to relax again. Now when
[28:09] they can learn to relax again. Now when it's really bad, you have to go see a
[28:11] it's really bad, you have to go see a pelvic floor physical therapist who can
[28:13] pelvic floor physical therapist who can really work with you to identify which
[28:14] really work with you to identify which of the muscles are maybe more
[28:16] of the muscles are maybe more dysfunctional and maybe work
[28:17] dysfunctional and maybe work specifically on those. Um but I think
[28:20] specifically on those. Um but I think it's it's so important. that's under
[28:21] it's it's so important. that's under evvaluated, underd discussed when it
[28:24] evvaluated, underd discussed when it comes to sexual function. It's a hugely
[28:26] comes to sexual function. It's a hugely important part of sexual function.
[28:29] important part of sexual function. >> How many people are struggling with
[28:31] >> How many people are struggling with these issues, specifically the like
[28:34] these issues, specifically the like erectile dysfunction issues and what age
[28:36] erectile dysfunction issues and what age are they?
[28:37] are they? >> Yeah. So, it starts early. I think
[28:39] >> Yeah. So, it starts early. I think there's always been a sort of a
[28:40] there's always been a sort of a disconnect where we think young guys
[28:42] disconnect where we think young guys don't have this or if they have it, it's
[28:43] don't have this or if they have it, it's all in their head. That's not
[28:45] all in their head. That's not necessarily true. Many young men do
[28:47] necessarily true. Many young men do develop erectile dysfunction because of
[28:49] develop erectile dysfunction because of biologic factors, but the data is really
[28:52] biologic factors, but the data is really robust on older guys. So above the age
[28:54] robust on older guys. So above the age of 50, we see 52% of men having erectile
[28:58] of 50, we see 52% of men having erectile dysfunction, which is
[28:59] dysfunction, which is >> 52%.
[29:01] >> 52%. >> And it goes up 10% every decade. So 60%
[29:05] >> And it goes up 10% every decade. So 60% of 60-y olds, 70% of 70 year olds. So
[29:07] of 60-y olds, 70% of 70 year olds. So erectile dysfunction continues to
[29:09] erectile dysfunction continues to worsen. This happens because one, you
[29:12] worsen. This happens because one, you know, our blood vessels get older, they
[29:14] know, our blood vessels get older, they get stiffer, they're not functioning as
[29:16] get stiffer, they're not functioning as well as they should. And we're seeing
[29:18] well as they should. And we're seeing also a rise in all these other comorbid
[29:20] also a rise in all these other comorbid conditions like diabetes, cholesterol,
[29:22] conditions like diabetes, cholesterol, high blood pressure. All these affect
[29:24] high blood pressure. All these affect how healthy your blood vessels are. And
[29:26] how healthy your blood vessels are. And so with these, they see the problem in
[29:29] so with these, they see the problem in their penis or in in women's case in
[29:31] their penis or in in women's case in their clitoris before they see heart
[29:33] their clitoris before they see heart disease or strokes or brain issues or
[29:35] disease or strokes or brain issues or dementia. And I think this is worth
[29:37] dementia. And I think this is worth pausing on which is a erectile
[29:40] pausing on which is a erectile dysfunction problem is a often a symptom
[29:43] dysfunction problem is a often a symptom of a cardiovascular issue. Right.
[29:45] of a cardiovascular issue. Right. >> Yeah. We call it a canary in a coal
[29:47] >> Yeah. We call it a canary in a coal mine. So it is like telling you that
[29:49] mine. So it is like telling you that something bad is coming. So the data
[29:51] something bad is coming. So the data would support that when you have
[29:53] would support that when you have erectile dysfunction if it's because of
[29:56] erectile dysfunction if it's because of an organic reason, right? not
[29:57] an organic reason, right? not psychoggenic but most people I think
[29:58] psychoggenic but most people I think have a combination of both that within 3
[30:01] have a combination of both that within 3 to 5 years you will start developing
[30:03] to 5 years you will start developing issues with your heart and so it
[30:06] issues with your heart and so it precedes those issues and if 7 years
[30:09] precedes those issues and if 7 years later 14% of those guys will have a
[30:12] later 14% of those guys will have a heart attack and so it's really an
[30:14] heart attack and so it's really an opportunity sexual problems are an
[30:17] opportunity sexual problems are an opportunity to look inside to figure out
[30:20] opportunity to look inside to figure out what's going on and to investigate and
[30:22] what's going on and to investigate and to change your life
[30:23] to change your life >> and when you say erectile dysfunction we
[30:25] >> and when you say erectile dysfunction we should probably define what that means
[30:26] should probably define what that means because there's going to be a lot of
[30:27] because there's going to be a lot of guys sat at home now thinking, "Oh my
[30:29] guys sat at home now thinking, "Oh my god, like my penis is a bit softer than
[30:31] god, like my penis is a bit softer than usual."
[30:31] usual." >> Yeah. So, erectile dysfunction is
[30:34] >> Yeah. So, erectile dysfunction is defined as the inability to maintain an
[30:36] defined as the inability to maintain an erection that's sufficient for
[30:38] erection that's sufficient for intercourse. So, you can get an
[30:40] intercourse. So, you can get an erection, but it goes away before you
[30:42] erection, but it goes away before you ejaculate or climax.
[30:44] ejaculate or climax. >> And so, that's typically and I think
[30:45] >> And so, that's typically and I think it's really important to differentiate
[30:46] it's really important to differentiate that from something like premature
[30:48] that from something like premature ejaculation where you climax too soon.

[31:11] occasions that he's lost his erection during sex. Is that erectile
[31:13] during sex. Is that erectile dysfunction? But what if he can keep his
[31:16] dysfunction? But what if he can keep his erection watching porn?
[31:18] erection watching porn? >> Well, so that's more multiffactorial,
[31:20] >> Well, so that's more multiffactorial, right? So I think if you lose it once or
[31:22] right? So I think if you lose it once or twice, right? Most guys will have an
[31:24] twice, right? Most guys will have an issue where they lose an erection at
[31:25] issue where they lose an erection at some point in their life. I think the
[31:26] some point in their life. I think the important thing is not to catastrophize.
[31:28] important thing is not to catastrophize. It's a problem when it becomes a
[31:30] It's a problem when it becomes a routine. Now, if someone is telling me,
[31:32] routine. Now, if someone is telling me, "Hey, I can watch porn and maintain my
[31:34] "Hey, I can watch porn and maintain my erection, but I can't with a partner."
[31:36] erection, but I can't with a partner." There's multiple different reasons for
[31:38] There's multiple different reasons for that that could be at play. One is that
[31:40] that that could be at play. One is that there's no pressure, right? You're by
[31:41] there's no pressure, right? You're by yourself. You're watching porn. There's
[31:43] yourself. You're watching porn. There's no pressure of performance. There's no
[31:45] no pressure of performance. There's no anxiety of performance. So, that may be
[31:46] anxiety of performance. So, that may be part of it. Other thing may be that that
[31:49] part of it. Other thing may be that that level of arousal that you're getting
[31:51] level of arousal that you're getting from porn. If you're watching like let's
[31:52] from porn. If you're watching like let's say the same kind of porn every time,
[31:54] say the same kind of porn every time, you're masturbating the same way every
[31:56] you're masturbating the same way every time, maybe using a firm grip or some
[31:58] time, maybe using a firm grip or some people will masturbate like uh facing
[32:00] people will masturbate like uh facing the bed or um you know against hard
[32:03] the bed or um you know against hard objects. And so that
[32:05] objects. And so that >> against hot objects.
[32:07] >> against hot objects. >> Yeah. Like the their fist or something.
[32:09] >> Yeah. Like the their fist or something. And so that u can't be replicated by a
[32:12] And so that u can't be replicated by a person, right? You can't replicate those

[32:23] that turns you on and that's the only thing that really gets you going and

[32:28] really get with your partner, then it will be difficult to reach the level of
[32:30] will be difficult to reach the level of arousal that you need to get an
[32:32] arousal that you need to get an erection.

[33:04] when you're a young boy, you get actually like erections three to five
[33:06] actually like erections three to five times a night, and they can last up to
[33:07] times a night, and they can last up to 40 minutes long. So you can spend a lot
[33:10] 40 minutes long. So you can spend a lot of your night with an erection. When you
[33:12] of your night with an erection. When you get older, like in your 40s, that drops
[33:14] get older, like in your 40s, that drops to about half the time. So maybe
[33:16] to about half the time. So maybe erections are a little shorter that they
[33:18] erections are a little shorter that they maybe max out at 30 minutes, but

[40:30] anything like that is I'm worried that it will cause a dopamine crash which
[40:32] it will cause a dopamine crash which will make me feel lazy and lethargic.

[40:48] feel a a sense of clarity. They call it postnut clarity, right? they feel clear,
[40:50] postnut clarity, right? they feel clear, they feel like motivated to do stuff,
[40:51] they feel like motivated to do stuff, they feel uh excited to go do something
[40:54] they feel uh excited to go do something else and they sort of move on from what
[40:55] else and they sort of move on from what they're doing. There's some people who
[40:56] they're doing. There's some people who feel post-coidal dysphoria. They
[40:58] feel post-coidal dysphoria. They actually feel sad or they feel depressed
[41:00] actually feel sad or they feel depressed after they orgasm. And we don't know
[41:03] after they orgasm. And we don't know exactly why this happens, but it is
[41:05] exactly why this happens, but it is because of this dramatic change in sort
[41:08] because of this dramatic change in sort of neurochemical signaling in the brain.
[41:10] of neurochemical signaling in the brain. And for those people who have it, it can
[41:12] And for those people who have it, it can be days where they sort of like feel
[41:14] be days where they sort of like feel bad. It can be hours where they feel
[41:16] bad. It can be hours where they feel sad. And so it can be really traumatic.
[41:18] sad. And so it can be really traumatic. So pillar two, what's pillar two of the

[41:43] you are chronically stressed, that means your cortisol is raised all the time,
[41:45] your cortisol is raised all the time, which is dampening your testosterone,
[41:47] which is dampening your testosterone, which is also keeping you in the
[41:49] which is also keeping you in the sympathetic state, and you just cannot
[41:51] sympathetic state, and you just cannot get in your mind space and in the
[41:54] get in your mind space and in the nervous system place to actually have
[41:56] nervous system place to actually have sex. So, that's super important. And I
[41:59] sex. So, that's super important. And I think, you know, there's lots of
[42:00] think, you know, there's lots of different ways to alleviate stress. You
[42:03] different ways to alleviate stress. You got to figure out what works for you.
[42:04] got to figure out what works for you. Um, for some people, using a stress
[42:06] Um, for some people, using a stress ball, even just at work. And I think
[42:08] ball, even just at work. And I think part of it you can actually incorporate
[42:10] part of it you can actually incorporate some intimacy. There's some evidence,
[42:11] some intimacy. There's some evidence, you know, the Gottmans have been on your
[42:13] you know, the Gottmans have been on your podcast doing a 20 second hug. So
[42:16] podcast doing a 20 second hug. So standing with your partner or loved one
[42:18] standing with your partner or loved one and hugging for 20 seconds on your own
[42:21] and hugging for 20 seconds on your own like independent gravity
[42:23] like independent gravity >> actually helps alleviate stress and
[42:25] >> actually helps alleviate stress and break sort of a stress loop. Doing a
[42:27] break sort of a stress loop. Doing a six-second kiss with your partner. And
[42:30] six-second kiss with your partner. And so it seems like six seconds, no big
[42:32] so it seems like six seconds, no big deal, but actually if you time it, it's
[42:33] deal, but actually if you time it, it's like hm if I'm just doing a kiss and I'm
[42:34] like hm if I'm just doing a kiss and I'm not like actually it's a little bit
[42:36] not like actually it's a little bit longer than normal. And so these sort of
[42:38] longer than normal. And so these sort of small things can help alleviate stress.

[42:59] someone. They're having a normal social interaction.
[43:01] interaction. So, and then being creative. And we've
[43:03] So, and then being creative. And we've been trying to work with this work on
[43:04] been trying to work with this work on this with our kids because there's so
[43:06] this with our kids because there's so much external stimuli all the time,
[43:07] much external stimuli all the time, right? They want to play video games,
[43:09] right? They want to play video games, they want to watch TV, they want to play
[43:10] they want to watch TV, they want to play sports, but we want them to be bored and
[43:13] sports, but we want them to be bored and be creative, like come up with creative
[43:14] be creative, like come up with creative ideas to keep themselves
[43:17] ideas to keep themselves interested and engaged in a different
[43:18] interested and engaged in a different way. So, I think that's so so important.
[43:21] way. So, I think that's so so important. And outside of stress, sleep. Um, sleep
[43:24] And outside of stress, sleep. Um, sleep is there's just so much abundance of
[43:26] is there's just so much abundance of data on how sleep affects hormonal
[43:28] data on how sleep affects hormonal health. They looked at data on men
[43:30] health. They looked at data on men sleeping 5 hours a night versus eight
[43:32] sleeping 5 hours a night versus eight hours a night. So you take the same guy
[43:34] hours a night. So you take the same guy and he sleeps eight hours, then you, you
[43:35] and he sleeps eight hours, then you, you know, do five five hours for a few days.
[43:38] know, do five five hours for a few days. His test for a week, his testosterone
[43:39] His test for a week, his testosterone drops by 15%.

[44:23] think. It's just erectile dysfunction, but it's this whole constellation of
[44:25] but it's this whole constellation of symptoms. It can also be increased fat
[44:27] symptoms. It can also be increased fat mass, decreased muscle mass. So all of
[44:29] mass, decreased muscle mass. So all of these things can happen because we have
[44:31] these things can happen because we have testosterone receptors all over our
[44:33] testosterone receptors all over our body. We have them in our brain. We have
[44:34] body. We have them in our brain. We have them in our muscle. We have them in our
[44:36] them in our muscle. We have them in our bone. And these can all have really
[44:38] bone. And these can all have really serious consequences when it's low. when
[44:41] serious consequences when it's low. when you have sleep apnnea. And how you know

[45:55] like phalates and BPAs which can affect hormonal health. They can mimic
[45:57] hormonal health. They can mimic estrogen. They can reduce the production
[46:00] estrogen. They can reduce the production of testosterone based on these
[46:02] of testosterone based on these mechanisms. Also things like plastics in

[46:31] in the sun for hours and hours and hours because as it gets warm, it releases
[46:33] because as it gets warm, it releases more microlastics and more chemicals
[46:35] more microlastics and more chemicals into the water. When you're eating food,
[46:37] into the water. When you're eating food, if you are eating out of say you get
[46:39] if you are eating out of say you get takeout, put it on a plate and warm it

[48:06] to keep people who can help you. But it also plays a role in your sexual
[48:08] also plays a role in your sexual environment, right? So my husband is in
[48:10] environment, right? So my husband is in these groups with a bunch of other male
[48:12] these groups with a bunch of other male physicians. And there's a lot of
[48:13] physicians. And there's a lot of discussion about how they never have sex
[48:15] discussion about how they never have sex anymore. And they're all just like, "Oh
[48:17] anymore. And they're all just like, "Oh yeah, this is just like normal. Like
[48:18] yeah, this is just like normal. Like we're getting older. Our partners don't
[48:20] we're getting older. Our partners don't want to have sex. It's so frustrating."
[48:21] want to have sex. It's so frustrating." And there's a lot of discussion about
[48:22] And there's a lot of discussion about this and and it's it's almost like, "Oh,
[48:26] this and and it's it's almost like, "Oh, that's okay." Like that's just normal,
[48:27] that's okay." Like that's just normal, right? They're normalizing this
[48:29] right? They're normalizing this experience where that sex has not become
[48:31] experience where that sex has not become a priority because life is so stressful,
[48:33] a priority because life is so stressful, things are so crazy. Maybe you're having
[48:34] things are so crazy. Maybe you're having relationship discord. So, it absolutely
[48:37] relationship discord. So, it absolutely plays a role. And there's actually some
[48:39] plays a role. And there's actually some evidence that it may help you make
[48:41] evidence that it may help you make better choices. So, there's one study
[48:42] better choices. So, there's one study where they looked at I think it was like
[48:44] where they looked at I think it was like 50,000 people and they um did an
[48:48] 50,000 people and they um did an intervention where they tried to
[48:49] intervention where they tried to encourage these people to surround
[48:52] encourage these people to surround themselves with positive influences that
[48:53] themselves with positive influences that were like have safe sex and avoid
[48:55] were like have safe sex and avoid negative influences. And they saw that
[48:57] negative influences. And they saw that for that these people had an increase in
[49:00] for that these people had an increase in like sexual positive behavior. So like
[49:02] like sexual positive behavior. So like safe sex practices by 46%.
[49:05] safe sex practices by 46%. So just by changing who they spent their
[49:08] So just by changing who they spent their time with, they saw this major
[49:10] time with, they saw this major difference in how they approached their
[49:12] difference in how they approached their sex lives.
[49:12] sex lives. >> Okay. So if your friends are very
[49:14] >> Okay. So if your friends are very pessimistic about sex and they're not
[49:15] pessimistic about sex and they're not having and they're complaining about it,
[49:17] having and they're complaining about it, that's going to become sort of
[49:19] that's going to become sort of contagious.
[49:20] contagious. >> Yeah. And I think it it extrapolates to
[49:22] >> Yeah. And I think it it extrapolates to everything. what kind of relationships
[49:24] everything. what kind of relationships you you're in. How do they view their
[49:26] you you're in. How do they view their relationship with their partner? Is it a
[49:28] relationship with their partner? Is it a priority? Do they make it a priority?
[49:30] priority? Do they make it a priority? Right? Or are they just busy doing their
[49:31] Right? Or are they just busy doing their own thing and they're just like living
[49:33] own thing and they're just like living in parallel lives? They're doing their
[49:35] in parallel lives? They're doing their own thing. You're doing your own thing.
[49:36] own thing. You're doing your own thing. You never really spend time with your
[49:37] You never really spend time with your partner and then when you go hang out
[49:39] partner and then when you go hang out with your friends, you're like, "Oh god,
[49:41] with your friends, you're like, "Oh god, I'm so glad to get away." And you're not
[49:42] I'm so glad to get away." And you're not like talking about how much you enjoy
[49:44] like talking about how much you enjoy your partner. People tend to talk
[49:47] your partner. People tend to talk negatively, right? They want to like
[49:48] negatively, right? They want to like sort of like vent. And so when you have
[49:50] sort of like vent. And so when you have people who don't make that a part of
[49:52] people who don't make that a part of their lives, it also affects you. They
[49:53] their lives, it also affects you. They value their partner and they talk about
[49:55] value their partner and they talk about them in a positive way, that's a a great
[49:58] them in a positive way, that's a a great thing to say like, okay, this is the
[49:59] thing to say like, okay, this is the right kind of person I want because I
[50:01] right kind of person I want because I want to value my partner too and I don't
[50:03] want to value my partner too and I don't want that to rub off on me because it
[50:05] want that to rub off on me because it absolutely does.
[50:06] absolutely does. >> And the other thing you said in this
[50:07] >> And the other thing you said in this category of environmental stuff is
[50:09] category of environmental stuff is things like pornography consumption. Is
[50:10] things like pornography consumption. Is it possible to consume too much porn?
[50:12] it possible to consume too much porn? >> So it's not the amount of porn that you
[50:14] >> So it's not the amount of porn that you watch, it is the sort of the way you
[50:17] watch, it is the sort of the way you feel about it. Many people watch porn
[50:19] feel about it. Many people watch porn for a variety of different reasons. Most
[50:21] for a variety of different reasons. Most often it's for pleasure and enjoyment.
[50:23] often it's for pleasure and enjoyment. But there's some people who watch porn
[50:24] But there's some people who watch porn because they want to get away from
[50:26] because they want to get away from negative feelings, right? This is giving
[50:28] negative feelings, right? This is giving them dopamine and they're using it as a
[50:30] them dopamine and they're using it as a way to just feel better in the moment.
[50:32] way to just feel better in the moment. And so that become that can become a
[50:35] And so that become that can become a compulsion where they're watching porn
[50:36] compulsion where they're watching porn and it's like giving them these positive
[50:38] and it's like giving them these positive feelings and they feel negative in life
[50:39] feelings and they feel negative in life and they're going back to porn to sort
[50:41] and they're going back to porn to sort of again have that compulsion to watch
[50:43] of again have that compulsion to watch porn again. But that's a small subset of
[50:45] porn again. But that's a small subset of people. The other thing is that there's
[50:47] people. The other thing is that there's a lot of negative discussion around
[50:49] a lot of negative discussion around porn, right? People like porn is bad,
[50:51] porn, right? People like porn is bad, it's dangerous, it's evil. And I think,
[50:53] it's dangerous, it's evil. And I think, you know, it's more nuanced than that.
[50:55] you know, it's more nuanced than that. But if you feel that porn is bad, every
[50:58] But if you feel that porn is bad, every time you watch porn, you feel negative,
[51:00] time you watch porn, you feel negative, you feel bad about yourself. That's
[51:02] you feel bad about yourself. That's where we see people having the most
[51:04] where we see people having the most dysfunction associated with it because
[51:05] dysfunction associated with it because they feel guilty. They could watch it
[51:07] they feel guilty. They could watch it once a year and they could feel super
[51:09] once a year and they could feel super guilty about it and it could affect
[51:10] guilty about it and it could affect their sexual function. They could see
[51:12] their sexual function. They could see themselves as a bad person. and they
[51:13] themselves as a bad person. and they could see themselves as wrong or
[51:15] could see themselves as wrong or immoral. And that's really when it
[51:17] immoral. And that's really when it becomes an issue. If you are finding
[51:19] becomes an issue. If you are finding yourself either compulsively watching
[51:21] yourself either compulsively watching pornography and using it as a scapegoat,
[51:24] pornography and using it as a scapegoat, maybe time to reevaluate, say, why am I
[51:26] maybe time to reevaluate, say, why am I doing this? How can I switch this out
[51:28] doing this? How can I switch this out for something else that makes me feel
[51:29] for something else that makes me feel better that might be healthy, like
[51:31] better that might be healthy, like exercise or going for walks or whatever
[51:33] exercise or going for walks or whatever that may be? And then if it's something
[51:35] that may be? And then if it's something that's really making you feel like bad
[51:38] that's really making you feel like bad or guilty because maybe that's how you
[51:40] or guilty because maybe that's how you were raised, maybe that's what you feel
[51:41] were raised, maybe that's what you feel about morality, then yeah, maybe like
[51:43] about morality, then yeah, maybe like you either need to work through that
[51:45] you either need to work through that because porn is maybe something that you
[51:47] because porn is maybe something that you still want to watch at times or you need
[51:49] still want to watch at times or you need to abstain. But usually those are it's
[51:51] to abstain. But usually those are it's very difficult to abstain for most
[51:53] very difficult to abstain for most people to abstain for prolonged periods
[51:55] people to abstain for prolonged periods of time. So I think it's it's something
[51:56] of time. So I think it's it's something you have to work through if you find
[51:58] you have to work through if you find yourself having these sort of feelings
[51:59] yourself having these sort of feelings about pornography or you're compulsively
[52:01] about pornography or you're compulsively watching it. But for most people, it's
[52:04] watching it. But for most people, it's adults. For most adults, it is fantasy.
[52:07] adults. For most adults, it is fantasy. It's a place where you can experience uh
[52:11] It's a place where you can experience uh see things that you may never do in real
[52:12] see things that you may never do in real life that may be enjoyable, that are fun
[52:15] life that may be enjoyable, that are fun to watch, that have allow you to feel
[52:16] to watch, that have allow you to feel arousal. We see that when couples watch
[52:19] arousal. We see that when couples watch porn together, they are more likely to
[52:21] porn together, they are more likely to be more satisfied in their relationship.
[52:22] be more satisfied in their relationship. And when there's a discord, like one
[52:24] And when there's a discord, like one person really doesn't like it or doesn't
[52:26] person really doesn't like it or doesn't use it, and the other one uses it a lot,
[52:27] use it, and the other one uses it a lot, that's where we see the problem. Is porn
[52:30] that's where we see the problem. Is porn consumption going to impact my intimacy
[52:33] consumption going to impact my intimacy with my partner?
[52:35] with my partner? >> So, it depends. I think a lot of people
[52:37] >> So, it depends. I think a lot of people watch it without an issue, right? They
[52:39] watch it without an issue, right? They watch
[52:39] watch >> if I masturbate and I ejaculate, I'm not
[52:42] >> if I masturbate and I ejaculate, I'm not going to then be able to ejaculate very
[52:43] going to then be able to ejaculate very quickly necessarily with my partner.

[53:01] they're watching a lot of porn every single day and their refractory period
[53:03] single day and their refractory period is maybe a day. Then they may actually
[53:05] is maybe a day. Then they may actually really have trouble ejaculating with
[53:07] really have trouble ejaculating with their partner because they've not given
[53:09] their partner because they've not given themselves that time.
[53:11] themselves that time. >> This has had probably the single biggest

[58:22] you know, three times a week and you enjoy it. But if you start having
[58:23] enjoy it. But if you start having vanilla ice cream every single day,
[58:25] vanilla ice cream every single day, you're kind of bored with it, right? And
[58:27] you're kind of bored with it, right? And the same goes for sex. Now having
[58:29] the same goes for sex. Now having routine in sex, knowing what you like,
[58:32] routine in sex, knowing what you like, um knowing what your partner likes, it
[58:33] um knowing what your partner likes, it can be very helpful, but it also is nice
[58:37] can be very helpful, but it also is nice to add novelty. So if you think about
[58:39] to add novelty. So if you think about you can extrapolate from the flow state.

[58:57] can't be so hard that you're going to get frustrated, right? And so you need
[58:59] get frustrated, right? And so you need to add a slight challenge to your sexual
[59:03] to add a slight challenge to your sexual encounter to get into that flow state.
[59:05] encounter to get into that flow state. And that's when you start having really
[59:07] And that's when you start having really great sex cuz you're like in you're so
[59:09] great sex cuz you're like in you're so totally immersed. you're feeling great

[01:00:09] symphysis. So this bone right here um is sort of like rubbing against their
[01:00:11] sort of like rubbing against their clitoris like this area the skin here is
[01:00:13] clitoris like this area the skin here is rubbing against the clitoris while
[01:00:14] rubbing against the clitoris while you're penetrating and that has been
[01:00:16] you're penetrating and that has been shown to increase orgasm rate and
[01:00:19] shown to increase orgasm rate and pleasure in sex. But really it's about
[01:00:21] pleasure in sex. But really it's about figuring out and trying different things
[01:00:23] figuring out and trying different things and realizing like that for example that
[01:00:24] and realizing like that for example that coil alignment technique is sort of
[01:00:26] coil alignment technique is sort of difficult like it's not that easy and
[01:00:27] difficult like it's not that easy and intuitive and so you have to sort of
[01:00:29] intuitive and so you have to sort of play around with it and figure out like
[01:00:31] play around with it and figure out like what is going to work uh best and yeah
[01:00:33] what is going to work uh best and yeah realizing it might not work. You might
[01:00:35] realizing it might not work. You might be like super awkward and like okay like
[01:00:37] be like super awkward and like okay like that's okay like it's not the end of the

[01:00:41] world if sex is not perfect every time because we're learning and we're playing
[01:00:42] because we're learning and we're playing and we're having fun. And so I think
[01:00:44] and we're having fun. And so I think that's really the key is like not
[01:00:45] that's really the key is like not letting yourself get so frustrated if
[01:00:47] letting yourself get so frustrated if something doesn't go exactly the way you
[01:00:49] something doesn't go exactly the way you envision it in your head.

[01:01:10] of fluid at the time of orgasm. Um usually it's clear, colorless, and it
[01:01:11] usually it's clear, colorless, and it can be quite voluminous. um it's coming
[01:01:14] can be quite voluminous. um it's coming from the urethra which is the pee hole
[01:01:16] from the urethra which is the pee hole essentially and remember I talked about
[01:01:18] essentially and remember I talked about those skins glands those skins glands
[01:01:21] those skins glands those skins glands are the homalogue of the male prostate
[01:01:23] are the homalogue of the male prostate and they have a little bit of fluid in
[01:01:24] and they have a little bit of fluid in them too typically when you have
[01:01:28] them too typically when you have stimulation um and squirting they will
[01:01:30] stimulation um and squirting they will release fluid from the bladder as well
[01:01:33] release fluid from the bladder as well as the skins glands that combines
[01:01:34] as the skins glands that combines together and and it's emitted through
[01:01:37] together and and it's emitted through through that

[01:01:50] into the bladder and they took women who said they were squirters and they had
[01:01:51] said they were squirters and they had them orgasm and they saw is there dye in
[01:01:54] them orgasm and they saw is there dye in the uh in the fluid and yeah, there was
[01:01:57] the uh in the fluid and yeah, there was dy in fluid. So, it's coming, it has to
[01:01:59] dy in fluid. So, it's coming, it has to come from the bladder. The skins glands
[01:02:00] come from the bladder. The skins glands only hold a small amount, but it's
[01:02:02] only hold a small amount, but it's chemically different. It's usually
[01:02:04] chemically different. It's usually clear, odorless. It doesn't smell like
[01:02:06] clear, odorless. It doesn't smell like pee. And so, there's some theories as to
[01:02:08] pee. And so, there's some theories as to why that might be. It may be that um
[01:02:11] why that might be. It may be that um when you're having sex, there's
[01:02:13] when you're having sex, there's different hormonal signals that change
[01:02:14] different hormonal signals that change the concentration of what's filtering
[01:02:16] the concentration of what's filtering through your kidneys so that it is a
[01:02:18] through your kidneys so that it is a little bit different in composition.

[01:02:21] There's some theories that when there's more estrogen around that it may change.

[01:02:32] it's absolutely coming from the urethra which is connected to the bladder as
[01:02:34] which is connected to the bladder as well as the steam glands. But I think
[01:02:35] well as the steam glands. But I think like this this whole discussion about
[01:02:37] like this this whole discussion about what it is, it doesn't matter. Like is
[01:02:39] what it is, it doesn't matter. Like is it pleasurable? Are you enjoying it?
[01:02:41] it pleasurable? Are you enjoying it? Great. If you squirt and you enjoy it,
[01:02:43] Great. If you squirt and you enjoy it, great. If you don't, that doesn't mean
[01:02:45] great. If you don't, that doesn't mean that there's something wrong and that
[01:02:46] that there's something wrong and that your partner hasn't orgasmed. The way to
[01:02:48] your partner hasn't orgasmed. The way to figure out if your partner orgasm is you
[01:02:50] figure out if your partner orgasm is you ask her, right? Either she tells you or

[01:02:54] you ask her. And sometimes it's obvious and sometimes it's not, but you know,

[01:03:08] they're not proven. Um, of course, the first is that squirting contains PSA and
[01:03:10] first is that squirting contains PSA and zinc, which are naturally antibacterial,
[01:03:13] zinc, which are naturally antibacterial, and ejaculating these fluids during or
[01:03:14] and ejaculating these fluids during or after sex may have evolved to flush the
[01:03:17] after sex may have evolved to flush the urethra and prevent UTI. Um, the logic
[01:03:19] urethra and prevent UTI. Um, the logic being in a pre antibiotic world, a woman
[01:03:22] being in a pre antibiotic world, a woman who could flush bacteria out of her
[01:03:24] who could flush bacteria out of her system after mating was more likely to
[01:03:25] system after mating was more likely to stay healthy and reproduce. That's one
[01:03:26] stay healthy and reproduce. That's one theory. The other reason,
[01:03:27] theory. The other reason, >> well, before you I want to talk about
[01:03:28] >> well, before you I want to talk about that really quickly. So, that's a really
[01:03:30] that really quickly. So, that's a really interesting theory. So one the reason
[01:03:32] interesting theory. So one the reason they emit PSA is because schemes glands
[01:03:35] they emit PSA is because schemes glands are the homalologue of the prostate. So
[01:03:38] are the homalologue of the prostate. So the prostate makes PSA which is prostate
[01:03:40] the prostate makes PSA which is prostate specific antigen.
[01:03:41] specific antigen. >> So this point this word homalogue again.
[01:03:42] >> So this point this word homalogue again. >> Yeah it's a so basically the prostate in
[01:03:44] >> Yeah it's a so basically the prostate in the male the same tissue when they're an
[01:03:47] the male the same tissue when they're an embryo becomes the prostate in the male
[01:03:48] embryo becomes the prostate in the male and becomes a skins glands in the
[01:03:50] and becomes a skins glands in the female. So that's why it's emitting PSA.

[01:03:54] That is an interesting theory because there are a subset of women who get UTI
[01:03:56] there are a subset of women who get UTI after sex. Not everybody but some do.
[01:03:59] after sex. Not everybody but some do. And it's it's not because of the
[01:04:01] And it's it's not because of the ejaculate or because of the male
[01:04:04] ejaculate or because of the male harboring some bacteria. It's because of
[01:04:05] harboring some bacteria. It's because of the actual thrusting of the penis. It's
[01:04:08] the actual thrusting of the penis. It's taking bacteria from the outside and
[01:04:09] taking bacteria from the outside and making it more easy for it to go through
[01:04:11] making it more easy for it to go through the urethra into the bladder. And women
[01:04:13] the urethra into the bladder. And women have a short urethra. And so I want just
[01:04:15] have a short urethra. And so I want just to debunk that myth that it's like
[01:04:17] to debunk that myth that it's like you're getting it from your partner or
[01:04:19] you're getting it from your partner or there's something wrong with you. It is
[01:04:20] there's something wrong with you. It is literally just anatomy. And so some
[01:04:22] literally just anatomy. And so some women do get more UTI after sex.
[01:04:25] women do get more UTI after sex. >> And so that is an interesting theory
[01:04:26] >> And so that is an interesting theory because maybe that's true. I don't know.
[01:04:28] because maybe that's true. I don't know. The second theory was in ancient
[01:04:30] The second theory was in ancient ancestors, the hormonal surge during
[01:04:32] ancestors, the hormonal surge during orgasm and ejaculation actually
[01:04:33] orgasm and ejaculation actually triggered the release of an egg. Humans
[01:04:36] triggered the release of an egg. Humans evolved to ovulate on a cycle now, but
[01:04:38] evolved to ovulate on a cycle now, but we kept the plumbing and the neuro
[01:04:40] we kept the plumbing and the neuro hormonal reflex. It's a happy accident,
[01:04:42] hormonal reflex. It's a happy accident, a biological vestage that no longer
[01:04:43] a biological vestage that no longer serves its original reproductive
[01:04:45] serves its original reproductive purpose, but remains because it isn't
[01:04:47] purpose, but remains because it isn't harmful for survival.

[01:04:53] thought that maybe orgasming um spasms orgasm help with fertility but

[01:05:02] you just said there which is the mate selection theory. The intense pleasure
[01:05:03] selection theory. The intense pleasure and psychological response of squirting
[01:05:05] and psychological response of squirting or or orgasm act as a reward system. It
[01:05:08] or or orgasm act as a reward system. It incentivizes women to seek out specific
[01:05:10] incentivizes women to seek out specific partners who provide high levels of
[01:05:12] partners who provide high levels of stimulation, potentially signaling a
[01:05:13] stimulation, potentially signaling a better genetic compatibility or a more
[01:05:16] better genetic compatibility or a more attentive male, which would help with
[01:05:18] attentive male, which would help with the survival of of offspring. Maybe
[01:05:21] the survival of of offspring. Maybe >> that makes sense.

[01:05:36] can be an erogynous zone. We talked a little bit about it when we talked about
[01:05:37] little bit about it when we talked about sensate focus, right? You can explore,
[01:05:39] sensate focus, right? You can explore, you can find areas that people find uh
[01:05:42] you can find areas that people find uh erogynous. So when you looked at data,
[01:05:44] erogynous. So when you looked at data, necks, nipples, lips, buttocks, inner
[01:05:48] necks, nipples, lips, buttocks, inner thighs, all these areas are considered
[01:05:50] thighs, all these areas are considered arogynous by most most people regardless
[01:05:53] arogynous by most most people regardless of their um sexual orientation,
[01:05:55] of their um sexual orientation, regardless of their cultural upbringing.
[01:05:57] regardless of their cultural upbringing. It seems to be pretty universal. There's
[01:05:59] It seems to be pretty universal. There's actually a famous paper about
[01:06:00] actually a famous paper about non-genital orgasms and like how people
[01:06:03] non-genital orgasms and like how people orgasm without any genital stimulation.
[01:06:06] orgasm without any genital stimulation. So lip orgasms, anal sex orgasms.
[01:06:09] So lip orgasms, anal sex orgasms. >> Is that possible?

[01:06:20] childirth. Like there's there have been these documented ways to orgasm and it's
[01:06:23] these documented ways to orgasm and it's because these areas can be very
[01:06:24] because these areas can be very erogynous and sometimes stimulating
[01:06:26] erogynous and sometimes stimulating multiple areas can like add more
[01:06:29] multiple areas can like add more erogynous interest and that's why you
[01:06:31] erogynous interest and that's why you see people like maybe in the BDSM
[01:06:33] see people like maybe in the BDSM community they'll they'll be using um
[01:06:35] community they'll they'll be using um nipple clamps while they're doing other
[01:06:37] nipple clamps while they're doing other things right so there's there's a whole
[01:06:38] things right so there's there's a whole bunch of areas that are erogynous and
[01:06:41] bunch of areas that are erogynous and most of the times people are not really
[01:06:43] most of the times people are not really spending much time on foreplay or
[01:06:46] spending much time on foreplay or stimulating these erogynous zones as
[01:06:48] stimulating these erogynous zones as much as they should Right? You you have
[01:06:50] much as they should Right? You you have a whole body to play with. And how do
[01:06:51] a whole body to play with. And how do you stimulate them? There's actually
[01:06:53] you stimulate them? There's actually evidence behind that. So on your skin,
[01:06:55] evidence behind that. So on your skin, we have these special fibers called C
[01:06:57] we have these special fibers called C tactile eference. When you stimulate
[01:06:59] tactile eference. When you stimulate these fibers, they help you feel
[01:07:01] these fibers, they help you feel pleasure, enjoyment, those sorts of
[01:07:03] pleasure, enjoyment, those sorts of things. And so there was a study where
[01:07:06] things. And so there was a study where they took 19 couples, a small study, it
[01:07:07] they took 19 couples, a small study, it was out of London, and they basically
[01:07:10] was out of London, and they basically told them to stimulate an arogynous zone
[01:07:12] told them to stimulate an arogynous zone and a non-erogynous zone. The
[01:07:13] and a non-erogynous zone. The non-erogynous zone being the forehead.
[01:07:15] non-erogynous zone being the forehead. And so they had the couples stimulated.
[01:07:16] And so they had the couples stimulated. And they told them stimulated at levels
[01:07:18] And they told them stimulated at levels of 18 cm/s
[01:07:21] of 18 cm/s uh in terms of how fast you're caressing
[01:07:23] uh in terms of how fast you're caressing the arm or or or body part and at 3 cm
[01:07:27] the arm or or or body part and at 3 cm per second. And what they found was
[01:07:28] per second. And what they found was those who stimulated at 3 cm per second
[01:07:31] those who stimulated at 3 cm per second had more sexual arousal had more
[01:07:33] had more sexual arousal had more pleasant stimulation compared to those
[01:07:35] pleasant stimulation compared to those who were stimulating at 18 cm per
[01:07:37] who were stimulating at 18 cm per second. Now this makes sense because
[01:07:39] second. Now this makes sense because those C tactile aphrant fibers respond
[01:07:42] those C tactile aphrant fibers respond very well to that slow gentle caress.

[01:09:06] that there's been a 300% increase in the United States in the last 10 years of
[01:09:08] United States in the last 10 years of testosterone prescriptions. And my
[01:09:10] testosterone prescriptions. And my friends are increasingly having
[01:09:11] friends are increasingly having conversation about testosterone, which
[01:09:12] conversation about testosterone, which we weren't having
[01:09:14] we weren't having >> even 5 years ago. It wasn't a
[01:09:15] >> even 5 years ago. It wasn't a conversation. And now the conversation
[01:09:17] conversation. And now the conversation I'm having amongst my friends is like,
[01:09:18] I'm having amongst my friends is like, is our testosterone high enough? Do we
[01:09:19] is our testosterone high enough? Do we need to go get supplements to increase
[01:09:21] need to go get supplements to increase our testosterone?
[01:09:22] our testosterone? >> Testosterone is declining. So when you
[01:09:24] >> Testosterone is declining. So when you look at testosterone levels from the
[01:09:26] look at testosterone levels from the 1990s, like late 1990s, the average
[01:09:29] 1990s, like late 1990s, the average level was around 600 and if you look at
[01:09:32] level was around 600 and if you look at data around two 2015, it was 450. So
[01:09:35] data around two 2015, it was 450. So there's been like a 25% decline in
[01:09:38] there's been like a 25% decline in testosterone. Now you're going to ask me
[01:09:39] testosterone. Now you're going to ask me why. One, we talked about some of the
[01:09:41] why. One, we talked about some of the endocrine disrupting chemicals. That's
[01:09:43] endocrine disrupting chemicals. That's part of it certainly. Two, there's a
[01:09:45] part of it certainly. Two, there's a significant rise in obesity.
[01:09:47] significant rise in obesity. Testosterone has aromatase. And
[01:09:49] Testosterone has aromatase. And testosterone converts to estrogen using
[01:09:51] testosterone converts to estrogen using this enzyme called aromatase. And so
[01:09:54] this enzyme called aromatase. And so when you have more fat mass, you have
[01:09:55] when you have more fat mass, you have more aromatase. And more testosterone is
[01:09:57] more aromatase. And more testosterone is being converted to estrogen. And so now
[01:10:00] being converted to estrogen. And so now you have less testosterone.

[01:10:23] it. So that's one. Two is we see a rise in diabetes and insulin resistance which
[01:10:26] in diabetes and insulin resistance which also causes a decrease in testosterone.
[01:10:30] also causes a decrease in testosterone. We're seeing a rise in ultrarocessed
[01:10:32] We're seeing a rise in ultrarocessed food intake and that doesn't have the
[01:10:36] food intake and that doesn't have the optimal nutrition that you need to
[01:10:38] optimal nutrition that you need to optimize testosterone. So we're getting
[01:10:41] optimize testosterone. So we're getting you know a high highly caloric which
[01:10:43] you know a high highly caloric which then leads to obesity. We're getting um
[01:10:46] then leads to obesity. We're getting um lack of healthy fats. We're you know all
[01:10:48] lack of healthy fats. We're you know all these things are super important for
[01:10:50] these things are super important for testosterone. The Mediterranean diet is
[01:10:52] testosterone. The Mediterranean diet is what has been the most studied and
[01:10:54] what has been the most studied and basically anti-inflammatory diet is what
[01:10:56] basically anti-inflammatory diet is what they found for testosterone. So trying
[01:10:58] they found for testosterone. So trying to just eat like whole foods,
[01:11:00] to just eat like whole foods, unprocessed foods as much as possible
[01:11:03] unprocessed foods as much as possible and minimizing the inflammatory foods.
[01:11:05] and minimizing the inflammatory foods. >> Does testo having high testosterone make
[01:11:07] >> Does testo having high testosterone make me more fertile? No, not necessarily.
[01:11:10] me more fertile? No, not necessarily. So, I think people inherently think that
[01:11:12] So, I think people inherently think that the higher your testosterone level is
[01:11:14] the higher your testosterone level is when you check your blood work, that
[01:11:16] when you check your blood work, that that's better. And that's not
[01:11:17] that's better. And that's not necessarily true. Now, everyone is
[01:11:20] necessarily true. Now, everyone is different. And what I can't tell you is
[01:11:22] different. And what I can't tell you is what your receptors look like, what your
[01:11:25] what your receptors look like, what your testosterone or androgen receptors look
[01:11:27] testosterone or androgen receptors look like, how sensitive are they to
[01:11:29] like, how sensitive are they to testosterone. And everyone's a little
[01:11:31] testosterone. And everyone's a little bit different. But when you look at the
[01:11:32] bit different. But when you look at the same guy or you look at two different
[01:11:34] same guy or you look at two different guys, their variability can be so much
[01:11:36] guys, their variability can be so much that a guy who's 900 can be normal and a
[01:11:38] that a guy who's 900 can be normal and a guy who's 500 can be normal because
[01:11:40] guy who's 500 can be normal because everyone has individual genetic
[01:11:41] everyone has individual genetic variations. It's how their cells how
[01:11:44] variations. It's how their cells how sensitive their cells are, how many
[01:11:46] sensitive their cells are, how many cells they have, you know, how many
[01:11:47] cells they have, you know, how many latig they have in their testicles. Um
[01:11:50] latig they have in their testicles. Um the CAG repeats, which are these DNA
[01:11:52] the CAG repeats, which are these DNA repeats on the receptor themselves.
[01:11:54] repeats on the receptor themselves. People who have more are less sensitive
[01:11:56] People who have more are less sensitive to the testosterone that's around. So
[01:11:58] to the testosterone that's around. So they need more testosterone to get the
[01:11:59] they need more testosterone to get the same result. Whereas people who have
[01:12:01] same result. Whereas people who have less repeats have more sensitivity so
[01:12:03] less repeats have more sensitivity so they they don't need as much
[01:12:05] they they don't need as much testosterone. And so everyone is
[01:12:06] testosterone. And so everyone is individual. And so that's why it's
[01:12:08] individual. And so that's why it's really important to understand how are
[01:12:09] really important to understand how are you feeling, right? It's not about
[01:12:11] you feeling, right? It's not about chasing a number. And so absolutely we
[01:12:14] chasing a number. And so absolutely we know that when your testosterone is low
[01:12:16] know that when your testosterone is low below 214 nanogs per deciliter that your
[01:12:20] below 214 nanogs per deciliter that your risk of mortality goes up by two. So you
[01:12:22] risk of mortality goes up by two. So you are going to double your risk of dying
[01:12:24] are going to double your risk of dying if you have low testosterone. But when
[01:12:26] if you have low testosterone. But when you go super high, super physiologic,
[01:12:29] you go super high, super physiologic, meaning like 1,800 or higher, now you're
[01:12:31] meaning like 1,800 or higher, now you're putting yourself at risk for other
[01:12:33] putting yourself at risk for other things. You can have blood thickening,
[01:12:35] things. You can have blood thickening, which is a known side effect of
[01:12:36] which is a known side effect of testosterone replacement, and that puts
[01:12:38] testosterone replacement, and that puts you at risk for stroke, heart attack,
[01:12:40] you at risk for stroke, heart attack, heart disease. Over long periods of
[01:12:43] heart disease. Over long periods of time, it can affect your heart because
[01:12:45] time, it can affect your heart because it can um there's also heart muscle
[01:12:48] it can um there's also heart muscle cells that get exposed to this high
[01:12:50] cells that get exposed to this high level of testosterone. And when that
[01:12:52] level of testosterone. And when that happens, these they sort of change over
[01:12:54] happens, these they sort of change over time and they become more collagen
[01:12:56] time and they become more collagen deposition. They get more fibrosis and
[01:12:59] deposition. They get more fibrosis and this makes them stiffer and so that
[01:13:00] this makes them stiffer and so that you're not pumping blood as effectively
[01:13:02] you're not pumping blood as effectively as you would otherwise. And so there are
[01:13:05] as you would otherwise. And so there are real consequences to going too high.
[01:13:08] real consequences to going too high. There's also the side effects of having
[01:13:09] There's also the side effects of having acne, of changes in mood in terms of
[01:13:12] acne, of changes in mood in terms of aggression and things when you get
[01:13:13] aggression and things when you get really high. So really, it's about
[01:13:15] really high. So really, it's about finding what you feel good at. More is
[01:13:18] finding what you feel good at. More is not necessarily better. Once your
[01:13:20] not necessarily better. Once your testosterone receptors are saturated,
[01:13:22] testosterone receptors are saturated, meaning they're all bound to
[01:13:23] meaning they're all bound to testosterone more doesn't help you. It
[01:13:25] testosterone more doesn't help you. It might help you get more muscles, which
[01:13:27] might help you get more muscles, which is why people abuse anabolic steroids
[01:13:29] is why people abuse anabolic steroids and testosterone because it will
[01:13:30] and testosterone because it will continue to help you get bigger muscles,
[01:13:32] continue to help you get bigger muscles, but that's it. It's not going to help
[01:13:34] but that's it. It's not going to help you with your brain health, your bone
[01:13:36] you with your brain health, your bone health, your sexual health, your any of
[01:13:38] health, your sexual health, your any of that.
[01:13:39] that. >> Is there a way to
[01:13:41] >> Is there a way to visually spot a person with low
[01:13:43] visually spot a person with low testosterone? Are there visual clues?
[01:13:46] testosterone? Are there visual clues? Usually it's someone who feels very

[01:14:06] the the organs right and so you don't see like a big pot belly necessarily. Um
[01:14:09] see like a big pot belly necessarily. Um they may have really low sexual desire.
[01:14:12] they may have really low sexual desire. They may have decrease in mood.
[01:14:13] They may have decrease in mood. >> What about skin and bones? Does it
[01:14:15] >> What about skin and bones? Does it impact skin and bones?
[01:14:16] impact skin and bones? >> Yeah. So, if you get high testosterone,
[01:14:18] >> Yeah. So, if you get high testosterone, you can get acne because it affects the
[01:14:19] you can get acne because it affects the sebum in the skin. Um, bone health. So,
[01:14:22] sebum in the skin. Um, bone health. So, testosterone converts to estradiol and
[01:14:26] testosterone converts to estradiol and estradiol is really important for bone
[01:14:28] estradiol is really important for bone metabolism. And so, when you have low
[01:14:31] metabolism. And so, when you have low testosterone, you can also have low
[01:14:33] testosterone, you can also have low estrogen. And when your estrogen gets
[01:14:35] estrogen. And when your estrogen gets very low, you get a higher risk for
[01:14:38] very low, you get a higher risk for fractures, higher risk for osteoporosis.
[01:14:41] fractures, higher risk for osteoporosis. And so that's where your bone health um
[01:14:43] And so that's where your bone health um can be in danger. And so that's why
[01:14:45] can be in danger. And so that's why testosterone can help protect your bones
[01:14:47] testosterone can help protect your bones because of its conversion to estradile.
[01:14:50] because of its conversion to estradile. >> If you do have low testosterone, should
[01:14:51] >> If you do have low testosterone, should you be considering taking anabolic
[01:14:53] you be considering taking anabolic steroids? Like
[01:14:55] steroids? Like and who is testosterone replacement
[01:14:57] and who is testosterone replacement therapy good for?

[01:15:08] people think anabolic steroids and testosterone replacement therapy are the
[01:15:09] testosterone replacement therapy are the same thing and they're not. And so
[01:15:11] same thing and they're not. And so testosterone replacement therapy is what
[01:15:14] testosterone replacement therapy is what is given to guys who have low
[01:15:16] is given to guys who have low testosterone. If you have truly low
[01:15:18] testosterone. If you have truly low testosterone, there's a few things you
[01:15:20] testosterone, there's a few things you can obviously improve testosterone
[01:15:21] can obviously improve testosterone naturally by going back to those pillars
[01:15:23] naturally by going back to those pillars of health that we talked about. Improve
[01:15:24] of health that we talked about. Improve your sleep, do resistance training,
[01:15:26] your sleep, do resistance training, avoid endocrine disrupting chemicals.
[01:15:28] avoid endocrine disrupting chemicals. But if you are still low despite doing
[01:15:30] But if you are still low despite doing all those things or you're like so
[01:15:32] all those things or you're like so fatigued that you can't move your body
[01:15:33] fatigued that you can't move your body and like you really need to do it, then
[01:15:35] and like you really need to do it, then I think testosterone replacement is is
[01:15:37] I think testosterone replacement is is very valuable. And so testosterone
[01:15:39] very valuable. And so testosterone replacement is getting you to a level of
[01:15:42] replacement is getting you to a level of testosterone that is within normal.
[01:15:45] testosterone that is within normal. We're not trying to get you super high.
[01:15:47] We're not trying to get you super high. We're not trying to get you to become a
[01:15:48] We're not trying to get you to become a bodybuilder. We're trying to get you to
[01:15:50] bodybuilder. We're trying to get you to normal, what you should be. And so
[01:15:53] normal, what you should be. And so anabolic steroids are different. And
[01:15:55] anabolic steroids are different. And anabol steroids are like all these
[01:15:56] anabol steroids are like all these different um oral pills and injectables
[01:15:58] different um oral pills and injectables and things that work on muscle as well
[01:16:00] and things that work on muscle as well as work on your uh androgen receptors.
[01:16:03] as work on your uh androgen receptors. And so those are typically things that
[01:16:05] And so those are typically things that people are taking on their own. They're
[01:16:08] people are taking on their own. They're kind of self-directing their care and
[01:16:10] kind of self-directing their care and they're not really monitoring their
[01:16:12] they're not really monitoring their testosterone or they don't really care
[01:16:14] testosterone or they don't really care how high they go. They just want the end
[01:16:16] how high they go. They just want the end product of looking muscular. And so they
[01:16:20] product of looking muscular. And so they are getting really high testosterones
[01:16:22] are getting really high testosterones and that has its own host of concerns in
[01:16:26] and that has its own host of concerns in addition to the ones we talked about
[01:16:27] addition to the ones we talked about like blood clots and and heart attack.
[01:16:29] like blood clots and and heart attack. But there's a 15 times higher risk of
[01:16:32] But there's a 15 times higher risk of having premature heart failure and a 122
[01:16:36] having premature heart failure and a 122 times more risk of cardiac death when
[01:16:39] times more risk of cardiac death when you're taking anabolic steroids for the
[01:16:41] you're taking anabolic steroids for the purposes of increasing muscle mass and
[01:16:43] purposes of increasing muscle mass and not and get getting really high in your
[01:16:45] not and get getting really high in your testosterone. So, it is a real serious
[01:16:48] testosterone. So, it is a real serious issue when you take anabolic steroids
[01:16:50] issue when you take anabolic steroids because it can have real serious health
[01:16:52] because it can have real serious health consequences.

[01:17:29] busy life. And so, I can guarantee your stress is probably not under control.
[01:17:31] stress is probably not under control. Like, probably you're not sleeping
[01:17:33] Like, probably you're not sleeping great. And not to say that those are
[01:17:35] great. And not to say that those are things that, you know, you can't do in
[01:17:36] things that, you know, you can't do in addition to taking testosterone if your
[01:17:38] addition to taking testosterone if your testosterone is indeed low. But I think
[01:17:41] testosterone is indeed low. But I think the important thing is realizing like
[01:17:43] the important thing is realizing like it's only going to help you if your
[01:17:45] it's only going to help you if your testosterone receptors are not fully
[01:17:47] testosterone receptors are not fully saturated and giving you more
[01:17:48] saturated and giving you more testosterone is going to saturate them.
[01:17:50] testosterone is going to saturate them. >> Doesn't it just drop anyway with age?
[01:17:52] >> Doesn't it just drop anyway with age? >> It does. So it drops about uh 1% a year
[01:17:56] >> It does. So it drops about uh 1% a year on average. Some people drop less, some
[01:17:58] on average. Some people drop less, some people drop more. So if you look at
[01:17:59] people drop more. So if you look at someone who is healthy and they have a
[01:18:02] someone who is healthy and they have a normal testosterone, they usually won't
[01:18:04] normal testosterone, they usually won't become what we call hypogonatal or have
[01:18:06] become what we call hypogonatal or have low testosterone even as they age
[01:18:09] low testosterone even as they age because the drop is so low. it's only 1%
[01:18:11] because the drop is so low. it's only 1% a year. But if you add on these comorbid
[01:18:14] a year. But if you add on these comorbid conditions, if you add on a high stress
[01:18:15] conditions, if you add on a high stress life, if you add on poor sleep, if you
[01:18:18] life, if you add on poor sleep, if you add on um exposure to endocrine
[01:18:20] add on um exposure to endocrine disrupting chemicals, you're going to
[01:18:21] disrupting chemicals, you're going to see um that number come down much
[01:18:24] see um that number come down much higher.

[01:18:44] at, 20 to 40% of guys have low testosterone. Um, and when you look at
[01:18:47] testosterone. Um, and when you look at the number that get treated, it's like
[01:18:48] the number that get treated, it's like 2%.

[01:19:02] normal and what's not is because you look at population-based data and you
[01:19:04] look at population-based data and you look at when these symptoms arise and
[01:19:06] look at when these symptoms arise and there are like clear numbers like you
[01:19:08] there are like clear numbers like you may start having bone loss uh around
[01:19:10] may start having bone loss uh around 300. You might start having uh you know
[01:19:14] 300. You might start having uh you know decreased sexual frequency of thoughts
[01:19:16] decreased sexual frequency of thoughts at 215. You might start having feeling
[01:19:19] at 215. You might start having feeling less physical vigor uh around 290.
[01:19:22] less physical vigor uh around 290. Right? And so there are numbers that we
[01:19:24] Right? And so there are numbers that we know based on population-based data that
[01:19:26] know based on population-based data that we're seeing these numbers decline. Now
[01:19:28] we're seeing these numbers decline. Now the other thing to think about when
[01:19:30] the other thing to think about when you're looking at testosterone is free
[01:19:31] you're looking at testosterone is free testosterone. So testosterone comes in
[01:19:34] testosterone. So testosterone comes in different forms in the body. It comes
[01:19:36] different forms in the body. It comes bound and unbound. So 45% of
[01:19:39] bound and unbound. So 45% of testosterone is bound to SHBG which is a
[01:19:43] testosterone is bound to SHBG which is a molecule of protein that is really
[01:19:45] molecule of protein that is really tightly binds to testosterone. Doesn't
[01:19:47] tightly binds to testosterone. Doesn't let it go for your body to use. Then
[01:19:49] let it go for your body to use. Then there's some that's bound to albumin
[01:19:51] there's some that's bound to albumin which is sort of loosely bound and then
[01:19:53] which is sort of loosely bound and then there's 2% that's free 1 to 2% that's
[01:19:56] there's 2% that's free 1 to 2% that's free and that's what we say is like the
[01:19:57] free and that's what we say is like the most available to your body for use and
[01:20:01] most available to your body for use and there's actually you know good data that
[01:20:02] there's actually you know good data that looks at free testosterone and the
[01:20:04] looks at free testosterone and the levels of free testosterone are
[01:20:06] levels of free testosterone are correlated with these symptoms as well
[01:20:08] correlated with these symptoms as well and so you do have to look at the big
[01:20:10] and so you do have to look at the big picture so I always check a total
[01:20:12] picture so I always check a total testosterone as well as an SHBG because
[01:20:14] testosterone as well as an SHBG because some people can have elevated SHBG
[01:20:17] some people can have elevated SHBG thyroid dysfunction due to aging, just
[01:20:20] thyroid dysfunction due to aging, just genetic variability, right? In women,
[01:20:22] genetic variability, right? In women, SHBG goes up when you take oral
[01:20:24] SHBG goes up when you take oral contraceptives and it stays up for life
[01:20:27] contraceptives and it stays up for life if you take oral contraceptives. So, it
[01:20:28] if you take oral contraceptives. So, it can affect their free testosterone. The
[01:20:30] can affect their free testosterone. The other thing to think about testosterone
[01:20:31] other thing to think about testosterone is it's for life. Once you take it, your
[01:20:33] is it's for life. Once you take it, your body stops making its own testosterone.
[01:20:36] body stops making its own testosterone. Many people over years will notice that
[01:20:37] Many people over years will notice that their testo their testicles get smaller
[01:20:40] their testo their testicles get smaller um because their body stops making its
[01:20:42] um because their body stops making its own testosterone. So, it's not something
[01:20:44] own testosterone. So, it's not something to take lightly. Now if you want say you
[01:20:46] to take lightly. Now if you want say you were low right now and you wanted to
[01:20:48] were low right now and you wanted to conceive there are ways where you can
[01:20:50] conceive there are ways where you can take other um other pharmaceuticals that
[01:20:53] take other um other pharmaceuticals that will naturally boost your own production
[01:20:55] will naturally boost your own production like hcg or clomid or enclomophene.
[01:20:58] like hcg or clomid or enclomophene. These work basically uh in different
[01:21:00] These work basically uh in different ways to either tell your brain that hey
[01:21:02] ways to either tell your brain that hey we need more testosterone or they send
[01:21:04] we need more testosterone or they send they mimic the signals that tell your
[01:21:06] they mimic the signals that tell your testicles to make more testosterone. And
[01:21:09] testicles to make more testosterone. And so there are ways to do it and you
[01:21:10] so there are ways to do it and you definitely need someone who's, you know,
[01:21:12] definitely need someone who's, you know, well-versed in managing hormone
[01:21:14] well-versed in managing hormone management. Uh, but there are things
[01:21:16] management. Uh, but there are things that you can do if you're low. But
[01:21:18] that you can do if you're low. But again, I don't think it's once you once
[01:21:20] again, I don't think it's once you once you start these things, you can't go I
[01:21:21] you start these things, you can't go I mean, you can get off of it, but it's
[01:21:23] mean, you can get off of it, but it's sort of a process and it's not something
[01:21:24] sort of a process and it's not something that everyone will do because you're
[01:21:26] that everyone will do because you're going to feel good on testosterone if
[01:21:28] going to feel good on testosterone if you're taking it. And then when you get
[01:21:29] you're taking it. And then when you get off it, because your body is like no
[01:21:31] off it, because your body is like no longer making its own, you're going to
[01:21:32] longer making its own, you're going to feel horrible. So, do you think most 50
[01:21:34] feel horrible. So, do you think most 50 plus year old men should be taking
[01:21:37] plus year old men should be taking testosterone
[01:21:39] testosterone replacement therapies?
[01:21:40] replacement therapies? >> Only if they're symptomatic and they're
[01:21:42] >> Only if they're symptomatic and they're low, right? So,
[01:21:42] low, right? So, >> would it make them the average man, the
[01:21:44] >> would it make them the average man, the average 50-year-old man, if this is the
[01:21:45] average 50-year-old man, if this is the average, feel better on a daily basis?
[01:21:48] average, feel better on a daily basis? >> Not necessarily. Because if they are
[01:21:50] >> Not necessarily. Because if they are normal, like I said, if their
[01:21:51] normal, like I said, if their testosterone receptors are already fully
[01:21:53] testosterone receptors are already fully saturated, it's not going to change,
[01:21:54] saturated, it's not going to change, right? It's not going to change their
[01:21:56] right? It's not going to change their it, like I said, it may make their
[01:21:58] it, like I said, it may make their muscles look bigger, but it's not going
[01:21:59] muscles look bigger, but it's not going to change their their cognition. It's
[01:22:01] to change their their cognition. It's not going to change their mood. It's not
[01:22:03] not going to change their mood. It's not going to change their sex drive. It's
[01:22:04] going to change their sex drive. It's not going to change their erections when
[01:22:06] not going to change their erections when their testosterone receptors are fully
[01:22:08] their testosterone receptors are fully saturated.
[01:22:09] saturated. >> And you know, in this graph, what is
[01:22:10] >> And you know, in this graph, what is showing the decline here? Has that got
[01:22:12] showing the decline here? Has that got anything to do with whether their
[01:22:14] anything to do with whether their testosterone levels are fully saturated?

[01:22:20] fully. >> That's why we talk about symptoms,
[01:22:21] >> That's why we talk about symptoms, right? Because that's how I can tell. I
[01:22:23] right? Because that's how I can tell. I think, you know, that's part of what
[01:22:25] think, you know, that's part of what being a doctor is, right? Talking to
[01:22:26] being a doctor is, right? Talking to your patients and figuring out what's
[01:22:28] your patients and figuring out what's going on with them and what their
[01:22:29] going on with them and what their symptoms are and making sure it's
[01:22:30] symptoms are and making sure it's nothing else, right? You can have low
[01:22:32] nothing else, right? You can have low thyroid and that can mimic what what uh
[01:22:35] thyroid and that can mimic what what uh what low testosterone feels like. You
[01:22:37] what low testosterone feels like. You can have a high prolactin because you
[01:22:38] can have a high prolactin because you might have a benign tumor in your brain
[01:22:41] might have a benign tumor in your brain that's secretreting prolactin and that
[01:22:43] that's secretreting prolactin and that can reduce your testosterone. So that's
[01:22:44] can reduce your testosterone. So that's the the reason to get a full evaluation
[01:22:46] the the reason to get a full evaluation to make sure there's nothing else that
[01:22:48] to make sure there's nothing else that we're missing that's causing you to have
[01:22:50] we're missing that's causing you to have these symptoms if your other levels are
[01:22:52] these symptoms if your other levels are normal.
[01:22:53] normal. >> Uh okay, that makes sense. So just
[01:22:54] >> Uh okay, that makes sense. So just because my testosterone levels would be
[01:22:56] because my testosterone levels would be low doesn't mean I necessar if I don't
[01:22:59] low doesn't mean I necessar if I don't if I'm not symptomatic doesn't mean that
[01:23:01] if I'm not symptomatic doesn't mean that I should necessarily just assume that I
[01:23:03] I should necessarily just assume that I need testosterone replacement therapy.

[01:23:05] >> Exactly. >> Okay. Yeah. I think I think most of my
[01:23:08] >> Okay. Yeah. I think I think most of my male friends are just assuming that once
[01:23:09] male friends are just assuming that once we hit 50 we're all on the testosterone.
[01:23:11] we hit 50 we're all on the testosterone. >> I mean look I think when if you need it
[01:23:14] >> I mean look I think when if you need it it's a great drug. It's a great option
[01:23:16] it's a great drug. It's a great option to improve your quality of life. It's
[01:23:19] to improve your quality of life. It's obviously preventing uh bone loss. It's
[01:23:22] obviously preventing uh bone loss. It's improving your longevity. it's uh
[01:23:24] improving your longevity. it's uh preventing hopefully having some benefit
[01:23:26] preventing hopefully having some benefit in terms of cardioabolic health and so
[01:23:28] in terms of cardioabolic health and so yeah absolutely if you need it but it's
[01:23:31] yeah absolutely if you need it but it's not something that like everyone should
[01:23:32] not something that like everyone should just take cuz why not right like it's a
[01:23:35] just take cuz why not right like it's a when you play with hormones it's serious
[01:23:36] when you play with hormones it's serious business you know
[01:23:38] business you know >> I guess a lot of men are self-conscious
[01:23:39] >> I guess a lot of men are self-conscious in a way that they don't often admit and
[01:23:41] in a way that they don't often admit and they want to have big muscles
[01:23:43] they want to have big muscles >> and they want to feel strong
[01:23:44] >> and they want to feel strong >> and this is the the problem I'm seeing
[01:23:45] >> and this is the the problem I'm seeing with younger guys right now is that
[01:23:48] with younger guys right now is that there's a lot of young kids taking
[01:23:50] there's a lot of young kids taking anabolics because there's so much
[01:23:52] anabolics because there's so much pressure to look a certain way, right?
[01:23:54] pressure to look a certain way, right? Because nowadays when how do you meet
[01:23:56] Because nowadays when how do you meet people on social media through like
[01:23:58] people on social media through like swiping and and all you see is what they
[01:24:00] swiping and and all you see is what they look like. And so there's this pressure
[01:24:02] look like. And so there's this pressure and like I'll see it at the gym like I'm
[01:24:03] and like I'll see it at the gym like I'm like at the gym and there's these young
[01:24:05] like at the gym and there's these young kids who I'm like there is no way you
[01:24:06] kids who I'm like there is no way you are naturally that built at that age,
[01:24:09] are naturally that built at that age, right? Like unless you've been lifting
[01:24:11] right? Like unless you've been lifting since you were like seven years old. And
[01:24:13] since you were like seven years old. And so and of course there are some. But it
[01:24:16] so and of course there are some. But it is serious because one, it causes
[01:24:18] is serious because one, it causes infertility, right? When you're on
[01:24:19] infertility, right? When you're on testosterone replacement after about 18
[01:24:23] testosterone replacement after about 18 18 months, 70% of people on testosterone
[01:24:25] 18 months, 70% of people on testosterone replacement will be infertile.
[01:24:27] replacement will be infertile. >> 17
[01:24:28] >> 17 >> 70 70%.
[01:24:30] >> 70 70%. >> 70.
[01:24:31] >> 70. >> And even as soon as like 10 weeks, we
[01:24:33] >> And even as soon as like 10 weeks, we see some people having drops in their
[01:24:36] see some people having drops in their sperm count. So, it's variable with how
[01:24:38] sperm count. So, it's variable with how quickly you're going to see impacts on
[01:24:40] quickly you're going to see impacts on your fertility, but it absolutely
[01:24:42] your fertility, but it absolutely happens. And it can it can come back for
[01:24:45] happens. And it can it can come back for the large majority of people, but how
[01:24:47] the large majority of people, but how long it takes depends on your age and
[01:24:49] long it takes depends on your age and how long you were on testosterone
[01:24:50] how long you were on testosterone replacement therapy. And in some cases,
[01:24:52] replacement therapy. And in some cases, it can take as long as 2 years to come
[01:24:54] it can take as long as 2 years to come back.

[01:25:05] biomarker of health. We've seen that people who have poor semen parameters
[01:25:08] people who have poor semen parameters they have higher rates of mortality and
[01:25:10] they have higher rates of mortality and they actually live when you look at age
[01:25:12] they actually live when you look at age like what age people die they die
[01:25:14] like what age people die they die younger compared to people and it's dose
[01:25:16] younger compared to people and it's dose dependent. So if you're have like normal
[01:25:20] dependent. So if you're have like normal uh the concentration of sperm and then
[01:25:22] uh the concentration of sperm and then it keeps going down and down and down.
[01:25:23] it keeps going down and down and down. If you look at the age it's like they
[01:25:25] If you look at the age it's like they live to 78 77 76 like you can actually
[01:25:28] live to 78 77 76 like you can actually see it come down except for people who
[01:25:30] see it come down except for people who have no sperm because those are usually
[01:25:31] have no sperm because those are usually genetic disorders. Those are live a
[01:25:33] genetic disorders. Those are live a little longer because it's not
[01:25:34] little longer because it's not necessarily due to cardioabolic issues.
[01:25:37] necessarily due to cardioabolic issues. Um, but it's basically a reflection of
[01:25:39] Um, but it's basically a reflection of your overall health.

[01:28:35] sons, right? And so I see it like they talk about their penis in such a way
[01:28:37] talk about their penis in such a way like very early in life like oh my god
[01:28:39] like very early in life like oh my god my penis is so big or it should be so
[01:28:41] my penis is so big or it should be so big or whatever and and there and it's
[01:28:44] big or whatever and and there and it's like this it becomes this like this
[01:28:46] like this it becomes this like this society thing about who's got a bigger
[01:28:48] society thing about who's got a bigger penis like it's very interesting but
[01:28:50] penis like it's very interesting but there's a lot of pressure to feel like
[01:28:53] there's a lot of pressure to feel like and and a lot of discussion around
[01:28:54] and and a lot of discussion around bigger is better right it's it's talked
[01:28:57] bigger is better right it's it's talked about in media it's subtly joked about
[01:28:59] about in media it's subtly joked about it is um everywhere you look people are
[01:29:02] it is um everywhere you look people are talking about like being wellendowed as
[01:29:04] talking about like being wellendowed as being better. But interestingly, on my
[01:29:06] being better. But interestingly, on my podcast, we interviewed the the guy who
[01:29:09] podcast, we interviewed the the guy who has the largest medically verified
[01:29:10] has the largest medically verified penis. It's I think it's like 13 or 14
[01:29:13] penis. It's I think it's like 13 or 14 inches in length. And um he actually has
[01:29:16] inches in length. And um he actually has a lot of trouble because it's so large
[01:29:18] a lot of trouble because it's so large that people don't want to have sex with
[01:29:20] that people don't want to have sex with him and it's uncomfortable for them. And
[01:29:22] him and it's uncomfortable for them. And so, like, it's not all roses and
[01:29:24] so, like, it's not all roses and sunshine on the other side. But the
[01:29:27] sunshine on the other side. But the reality is that many people feel what's
[01:29:29] reality is that many people feel what's called small penis anxiety. and they
[01:29:31] called small penis anxiety. and they they even if they have a normalsized
[01:29:33] they even if they have a normalsized penis, they feel consumed with how the
[01:29:36] penis, they feel consumed with how the size of their penis is. And so that's
[01:29:38] size of their penis is. And so that's why this video did so well. I naively
[01:29:40] why this video did so well. I naively had no idea how many people it would
[01:29:42] had no idea how many people it would resonate with because I'm not a guy,
[01:29:43] resonate with because I'm not a guy, right? And I see patients all the time,
[01:29:45] right? And I see patients all the time, but at that time in my career, people
[01:29:47] but at that time in my career, people weren't coming to me to talk about it.
[01:29:48] weren't coming to me to talk about it. And so I um I realized like there's so
[01:29:52] And so I um I realized like there's so many people quietly feeling insecure.
[01:29:54] many people quietly feeling insecure. And we talk a lot about women having
[01:29:55] And we talk a lot about women having body image issues. Men do too, right?
[01:29:58] body image issues. Men do too, right? They feel insecure about the size of
[01:30:00] They feel insecure about the size of their penis because also they're seeing
[01:30:02] their penis because also they're seeing guys on pornography who have very
[01:30:05] guys on pornography who have very well-endowed fallaces and you know they
[01:30:07] well-endowed fallaces and you know they chose to be porn actors for a reason,
[01:30:09] chose to be porn actors for a reason, right? Because they are naturally
[01:30:11] right? Because they are naturally wellendowed. And so you're comparing
[01:30:13] wellendowed. And so you're comparing yourself to the outliers and the average
[01:30:15] yourself to the outliers and the average penis is about 5.3 to 5.5 in erect. But
[01:30:19] penis is about 5.3 to 5.5 in erect. But when you look at like what do women
[01:30:21] when you look at like what do women want, they will say on average they want
[01:30:23] want, they will say on average they want a 6 in penis. But when you look at like
[01:30:26] a 6 in penis. But when you look at like the kind of sex toys they buy and they
[01:30:28] the kind of sex toys they buy and they did this study because they were looking
[01:30:29] did this study because they were looking at people who they were trying to decide
[01:30:32] at people who they were trying to decide what kind of when they were developing

[01:32:03] that when you have the tiniest difference on your penis, you notice it,
[01:32:05] difference on your penis, you notice it, right? Like I have guys come to like the
[01:32:06] right? Like I have guys come to like the tiniest little thing on their penis and
[01:32:08] tiniest little thing on their penis and they're like, "Is this okay?" Like this
[01:32:09] they're like, "Is this okay?" Like this is new. And like luckily it's nothing,
[01:32:12] is new. And like luckily it's nothing, right? But I'm like they notice it. You
[01:32:14] right? But I'm like they notice it. You notice it when something's wrong. So

[01:35:27] seen plenty of patients who have now developed erectile dysfunction after
[01:35:29] developed erectile dysfunction after doing jelking because they've now
[01:35:30] doing jelking because they've now created damage to their penis. And so I
[01:35:34] created damage to their penis. And so I I really caution people because this
[01:35:36] I really caution people because this there is so much despair around sexual
[01:35:39] there is so much despair around sexual function, around penal health that
[01:35:42] function, around penal health that people are willing to try things and
[01:35:44] people are willing to try things and they might really hurt themselves. No
[01:35:46] they might really hurt themselves. No joke, I had a patient the other day ask
[01:35:48] joke, I had a patient the other day ask me if he should drink hydrogen peroxide
[01:35:50] me if he should drink hydrogen peroxide because he saw some ad that said
[01:35:52] because he saw some ad that said hydrogen peroxide will fix your
[01:35:54] hydrogen peroxide will fix your erections and I said please don't. um
[01:35:57] erections and I said please don't. um this is you know but he really was like
[01:36:00] this is you know but he really was like seriously earnestly asking me this
[01:36:02] seriously earnestly asking me this question and I you know I didn't want to
[01:36:04] question and I you know I didn't want to shame him by any means and I was like no
[01:36:05] shame him by any means and I was like no you know this is actually not safe and
[01:36:07] you know this is actually not safe and it's not going to help you and there's
[01:36:09] it's not going to help you and there's no pill there's no drink there's no
[01:36:11] no pill there's no drink there's no anything that's going to give you a
[01:36:13] anything that's going to give you a longer penis

[01:36:28] these surgeries to lengthen their penis and then they have a bad outcome and now
[01:36:31] and then they have a bad outcome and now their penis is disfigured forever. And
[01:36:33] their penis is disfigured forever. And that's what really um you never forget
[01:36:36] that's what really um you never forget that because that person knowing the
[01:36:38] that because that person knowing the risks and benefits perhaps made a choice
[01:36:40] risks and benefits perhaps made a choice and that choice didn't go the way they
[01:36:42] and that choice didn't go the way they wanted and now they have to live with
[01:36:43] wanted and now they have to live with that for the rest of their life and
[01:36:45] that for the rest of their life and that's really sad.

[01:38:06] data, are very happy with the size of their partner, are very content with
[01:38:09] their partner, are very content with average or slightly above average or
[01:38:10] average or slightly above average or slightly below average as long as they
[01:38:12] slightly below average as long as they are feeling intimate and they're feeling
[01:38:14] are feeling intimate and they're feeling pleasure. And so I think that there's a
[01:38:16] pleasure. And so I think that there's a misnomer that that people think if I
[01:38:18] misnomer that that people think if I have a longer penis I'm going to be able
[01:38:20] have a longer penis I'm going to be able to pleasure my partner more
[01:38:22] to pleasure my partner more >> as it relates to women. Is there

[01:38:49] speaking, knowing that like you will be able to fit in the average woman and you

[01:38:58] penetration is is not as important. In fact, only about 85% of women orgasm

[01:39:03] through penetration alone. They need clitoreral stimulation to achieve
[01:39:04] clitoreral stimulation to achieve climax. And so again, penetration feels
[01:39:07] climax. And so again, penetration feels good. I I tell guys, it's like somebody
[01:39:10] good. I I tell guys, it's like somebody rubbing your testicles or your
[01:39:12] rubbing your testicles or your paranneeium. feels good, but it's not
[01:39:14] paranneeium. feels good, but it's not going to make you orgasm. And so
[01:39:16] going to make you orgasm. And so penetrating, yeah, if you're getting a
[01:39:18] penetrating, yeah, if you're getting a lot of clitoreral stimulation because
[01:39:20] lot of clitoreral stimulation because maybe she has a thinner, you know, her
[01:39:22] maybe she has a thinner, you know, her clitoreral shaft is closer to the
[01:39:24] clitoreral shaft is closer to the vaginal canal, maybe, you know, the size

[01:40:11] where I think some people get worried when I say, "Oh, you need to relax your
[01:40:12] when I say, "Oh, you need to relax your pelvic floor." They're like, "Oh, I
[01:40:13] pelvic floor." They're like, "Oh, I don't want to be looser." Right? And
[01:40:16] don't want to be looser." Right? And that's not going to happen if you have a
[01:40:17] that's not going to happen if you have a normally functioning pelvic floor. If
[01:40:19] normally functioning pelvic floor. If you're
[01:40:19] you're >> So, the penis is going in here, right?

[01:41:05] different. It may be the way that you're moving in the vaginal canal and that may
[01:41:08] moving in the vaginal canal and that may be why it feels different.

[01:41:34] least the Japanese population that they studied was correlated with the length
[01:41:35] studied was correlated with the length of the penis.

[01:42:12] we see improvements in metabolic diseases. They have improvements in
[01:42:14] diseases. They have improvements in diabetes, improvements in heart disease,
[01:42:16] diabetes, improvements in heart disease, improvements in overall health, right?
[01:42:18] improvements in overall health, right? And so that is going to mean that your
[01:42:20] And so that is going to mean that your blood vessels are healthier. You're more
[01:42:22] blood vessels are healthier. You're more able to get blood flow to your genitals.
[01:42:24] able to get blood flow to your genitals. You're going to have stronger sexual
[01:42:26] You're going to have stronger sexual function and arousal, right? So that's
[01:42:29] function and arousal, right? So that's great. The other thing is that you are
[01:42:31] great. The other thing is that you are losing weight. So you feel better. you
[01:42:33] losing weight. So you feel better. you feel more like your body image is is
[01:42:36] feel more like your body image is is feeling good about yourself. Also, for
[01:42:38] feeling good about yourself. Also, for men, remember I talked about this fat
[01:42:40] men, remember I talked about this fat pad right above your penis.
[01:42:42] pad right above your penis. >> That's getting smaller. And so now you
[01:42:44] >> That's getting smaller. And so now you can see more of your penis. So you may
[01:42:46] can see more of your penis. So you may have heard of ompic penis where people
[01:42:47] have heard of ompic penis where people are saying, "Oh, my penis is getting
[01:42:48] are saying, "Oh, my penis is getting longer." It's not actually that your
[01:42:50] longer." It's not actually that your penis is getting longer. It's that this
[01:42:51] penis is getting longer. It's that this fat is going down. So now you're seeing
[01:42:53] fat is going down. So now you're seeing more of your penis,
[01:42:54] more of your penis, >> which makes it look longer.
[01:42:56] >> which makes it look longer. >> Yes. Exactly. Reality. Yes. Exactly.

[01:43:05] great things and I think that's wonderful. Now the other part of it is
[01:43:07] wonderful. Now the other part of it is ompic works on the brain and it works on
[01:43:09] ompic works on the brain and it works on the reward pathway. So you now see food
[01:43:12] the reward pathway. So you now see food and you don't feel like it's that much
[01:43:13] and you don't feel like it's that much of a reward. So you don't chase after
[01:43:15] of a reward. So you don't chase after you have less desire for food. There's
[01:43:17] you have less desire for food. There's emerging data that we're seeing people
[01:43:18] emerging data that we're seeing people on these medications, these GLP1s, have
[01:43:21] on these medications, these GLP1s, have less desire for, let's say, gambling,
[01:43:23] less desire for, let's say, gambling, let's say shopping, let's say, you know,
[01:43:25] let's say shopping, let's say, you know, alcohol, smoking, whatever it is, right?
[01:43:28] alcohol, smoking, whatever it is, right? And so there's a theoretical because it
[01:43:30] And so there's a theoretical because it works on the same pathways that it could
[01:43:32] works on the same pathways that it could also decrease desire for sex. And I
[01:43:35] also decrease desire for sex. And I think the thing that, you know, there's
[01:43:36] think the thing that, you know, there's so many people on these medications,
[01:43:38] so many people on these medications, there's so many metabolic benefits that
[01:43:40] there's so many metabolic benefits that I worry that people may not even realize
[01:43:42] I worry that people may not even realize that their sexual desire is changing,
[01:43:45] that their sexual desire is changing, right? They might just be like, "Oh, you
[01:43:47] right? They might just be like, "Oh, you know, I'm I'm eating less. I look great.
[01:43:49] know, I'm I'm eating less. I look great. Everything's wonderful." But like slowly
[01:43:50] Everything's wonderful." But like slowly in the background, they're like, "Oh,
[01:43:52] in the background, they're like, "Oh, maybe my desire is less." And maybe it's
[01:43:54] maybe my desire is less." And maybe it's creating discord in the relationship.
[01:43:56] creating discord in the relationship. Hard to say because we don't have data
[01:43:57] Hard to say because we don't have data yet. So yeah, I think it's just
[01:43:59] yet. So yeah, I think it's just something that you should be aware of
[01:44:00] something that you should be aware of that if you start feeling like you have
[01:44:03] that if you start feeling like you have less desire for sex or maybe you're
[01:44:05] less desire for sex or maybe you're like, "Man, I just really don't feel
[01:44:06] like, "Man, I just really don't feel into anything." Talk to your doctor cuz
[01:44:09] into anything." Talk to your doctor cuz maybe your dose is just too high and
[01:44:11] maybe your dose is just too high and maybe it just needs to come down a
[01:44:12] maybe it just needs to come down a little bit. And again, we don't know
[01:44:14] little bit. And again, we don't know yet. But I do worry about this because I
[01:44:16] yet. But I do worry about this because I feel like we're we're gonna start seeing
[01:44:18] feel like we're we're gonna start seeing it.

[01:44:24] >> Absolutely. So you can take people starting Ozmpi and you can say, "Hey,

[01:44:29] here is or or any GLP1." You know, there's questionnaires. So like for
[01:44:30] there's questionnaires. So like for women there's the female sexual function
[01:44:32] women there's the female sexual function index. For men there's Adam, which is a
[01:44:33] index. For men there's Adam, which is a questionnaire about testosterone. But
[01:44:35] questionnaire about testosterone. But you could there's sexual desire
[01:44:36] you could there's sexual desire questionnaires that you can use. Um, and
[01:44:38] questionnaires that you can use. Um, and you can take it at the beginning and
[01:44:39] you can take it at the beginning and then you can take it as they increase
[01:44:41] then you can take it as they increase their dose and see if there's a change,
[01:44:43] their dose and see if there's a change, right, in terms of their sexual desire.

[01:44:56] >> Yeah. I I wish I could say there was like a script that you can follow and
[01:44:57] like a script that you can follow and it's going to work. But everyone's so
[01:44:59] it's going to work. But everyone's so different. But I think the important
[01:45:00] different. But I think the important thing is like not giving up because what
[01:45:03] thing is like not giving up because what happens is because we don't ever talk
[01:45:05] happens is because we don't ever talk about sex, when you bring it up, it's
[01:45:07] about sex, when you bring it up, it's immediately like, "Oh my god, is
[01:45:08] immediately like, "Oh my god, is something wrong? Did I do something
[01:45:09] something wrong? Did I do something wrong? Um, do you not like me? Am I not
[01:45:11] wrong? Um, do you not like me? Am I not attractive?" Right? You're automatically
[01:45:13] attractive?" Right? You're automatically going down the rabbit hole of worry and
[01:45:15] going down the rabbit hole of worry and concern. And instead of dealing with it,
[01:45:17] concern. And instead of dealing with it, you're supposed to shut it down. No, I
[01:45:18] you're supposed to shut it down. No, I don't want to talk about it. Right? Um
[01:45:19] don't want to talk about it. Right? Um especially if like maybe you haven't
[01:45:21] especially if like maybe you haven't connected intimately in a while and like
[01:45:23] connected intimately in a while and like you just don't want to deal with that or
[01:45:24] you just don't want to deal with that or you're you have other stressors in life.
[01:45:27] you're you have other stressors in life. Maybe you're not feeling like very good
[01:45:29] Maybe you're not feeling like very good about yourself and so you're like,
[01:45:30] about yourself and so you're like, "Well, I don't really feel sexy, so I
[01:45:32] "Well, I don't really feel sexy, so I don't want to have sex." And so, I think
[01:45:34] don't want to have sex." And so, I think really understanding that whatever
[01:45:36] really understanding that whatever reaction you get from your partner when
[01:45:37] reaction you get from your partner when you do talk to them, it's not about you.
[01:45:39] you do talk to them, it's not about you. It's just usually about how they're
[01:45:40] It's just usually about how they're feeling. And so, don't stress about
[01:45:42] feeling. And so, don't stress about that. And two, like it's anything that
[01:45:45] that. And two, like it's anything that is worth having requires work. So having
[01:45:48] is worth having requires work. So having a good sex life, having intimacy over
[01:45:51] a good sex life, having intimacy over the long haul with someone, if you are
[01:45:54] the long haul with someone, if you are committed to that relationship, it is
[01:45:56] committed to that relationship, it is worth working for. It is worth dealing
[01:45:58] worth working for. It is worth dealing with a little bit of discord to get
[01:46:00] with a little bit of discord to get there. And so I tell people when you
[01:46:02] there. And so I tell people when you want to talk about sex, first of all,
[01:46:03] want to talk about sex, first of all, don't do it in the bedroom. Don't do it
[01:46:05] don't do it in the bedroom. Don't do it right before or after sex. Do it at a
[01:46:07] right before or after sex. Do it at a time where you are both calm, relaxed.
[01:46:09] time where you are both calm, relaxed. Maybe you're out for a walk, maybe
[01:46:10] Maybe you're out for a walk, maybe you're in the car, so you're not like
[01:46:12] you're in the car, so you're not like looking directly at each other. You're
[01:46:13] looking directly at each other. You're kind of parallel. And always start with
[01:46:15] kind of parallel. And always start with a positive thing. like I you know I
[01:46:17] a positive thing. like I you know I really enjoy this about our relationship
[01:46:19] really enjoy this about our relationship and I would love if we could talk about
[01:46:21] and I would love if we could talk about this and and maybe they're going to be
[01:46:23] this and and maybe they're going to be like uh I'm not ready for that and you
[01:46:25] like uh I'm not ready for that and you say maybe we could come up with a time
[01:46:27] say maybe we could come up with a time where we can talk about this that works
[01:46:28] where we can talk about this that works for you right let them feel like they're
[01:46:30] for you right let them feel like they're coming to the conversation with like
[01:46:32] coming to the conversation with like they're not being ambushed or like
[01:46:34] they're not being ambushed or like talking about something and then be
[01:46:35] talking about something and then be curious. I think the big thing is like
[01:46:37] curious. I think the big thing is like being curious like what's going on with
[01:46:39] being curious like what's going on with you like I want to I want to I want to
[01:46:41] you like I want to I want to I want to be there for you. I love you. you know,
[01:46:43] be there for you. I love you. you know, I miss what we used to have or I miss
[01:46:45] I miss what we used to have or I miss this about us. And I think that there's
[01:46:47] this about us. And I think that there's always these misconceptions. I hear it
[01:46:49] always these misconceptions. I hear it all the time, right? It'll be like this
[01:46:51] all the time, right? It'll be like this partner only wants sex and this partner
[01:46:53] partner only wants sex and this partner never wants sex and you know, and that's
[01:46:56] never wants sex and you know, and that's it. Like that's the end of the
[01:46:57] it. Like that's the end of the conversation. But the reality is that
[01:46:59] conversation. But the reality is that there it's it's somewhere in between. So
[01:47:01] there it's it's somewhere in between. So typically women tend to view sex as like
[01:47:05] typically women tend to view sex as like added stress sometimes when they're
[01:47:06] added stress sometimes when they're already stressed. Like they're like,
[01:47:07] already stressed. Like they're like, "Oh, it's just one more thing to do."
[01:47:10] "Oh, it's just one more thing to do." Whereas men look at it as a stress
[01:47:11] Whereas men look at it as a stress relief. So, one, you're coming at it
[01:47:13] relief. So, one, you're coming at it from two different angles. The other
[01:47:15] from two different angles. The other thing is men aren't really taught how to
[01:47:17] thing is men aren't really taught how to be intimate. And so, for a lot of men,
[01:47:20] be intimate. And so, for a lot of men, sex is their version of intimacy. Like,
[01:47:22] sex is their version of intimacy. Like, that's how they connect. That's how they
[01:47:24] that's how they connect. That's how they feel connected with a partner. And so,
[01:47:27] feel connected with a partner. And so, often times when they're doing these
[01:47:29] often times when they're doing these initiations or attempts at sex, it feels
[01:47:31] initiations or attempts at sex, it feels like they're being greedy or it feels
[01:47:32] like they're being greedy or it feels like they're they just want sex. And I
[01:47:34] like they're they just want sex. And I think we have to reframe how we look at
[01:47:36] think we have to reframe how we look at that. Like, sex is not just sex. It is
[01:47:38] that. Like, sex is not just sex. It is intimacy. It is being together
[01:47:40] intimacy. It is being together connecting with another human being. So
[01:47:42] connecting with another human being. So how do we do that right? And like how do
[01:47:45] how do we do that right? And like how do we make that sex worth having? We get
[01:47:48] we make that sex worth having? We get into this frame where we have sex as
[01:47:50] into this frame where we have sex as like in a hurried quick like just got to
[01:47:53] like in a hurried quick like just got to get this done way. I just need to get an
[01:47:54] get this done way. I just need to get an orgasm. I just need to get a release.
[01:47:56] orgasm. I just need to get a release. But sex is supposed to be something we
[01:47:58] But sex is supposed to be something we look forward to. It's supposed to be
[01:47:59] look forward to. It's supposed to be something that we're excited about. And
[01:48:01] something that we're excited about. And so we have to build that in. We have to
[01:48:02] so we have to build that in. We have to sort of like build the environment where
[01:48:05] sort of like build the environment where sex can can happen. So, it may mean
[01:48:08] sex can can happen. So, it may mean like, "Hey, we are affectionate during
[01:48:10] like, "Hey, we are affectionate during the day. Maybe we send a lovey text
[01:48:12] the day. Maybe we send a lovey text message. Maybe um you know, I give you a
[01:48:14] message. Maybe um you know, I give you a hug. I caress your back. I um tell you
[01:48:17] hug. I caress your back. I um tell you you look beautiful. I I'm I'm showing
[01:48:18] you look beautiful. I I'm I'm showing you gratitude for how much I care about
[01:48:20] you gratitude for how much I care about you in other ways." I think just really
[01:48:23] you in other ways." I think just really taking the time to understand why your
[01:48:25] taking the time to understand why your partner feels the way they do.
[01:48:26] partner feels the way they do. >> I'm curious about on that communication
[01:48:28] >> I'm curious about on that communication point about how different genders
[01:48:31] point about how different genders in heterosexual couples have different
[01:48:33] in heterosexual couples have different sort of taboos and things that are just
[01:48:35] sort of taboos and things that are just a bit sensitive. Um, and I was looking
[01:48:37] a bit sensitive. Um, and I was looking at the the variance between men and
[01:48:39] at the the variance between men and women, and the the top one for men was
[01:48:41] women, and the the top one for men was around performance anxiety. So, like
[01:48:43] around performance anxiety. So, like admitting to things like erectile
[01:48:44] admitting to things like erectile dysfunction because it feels like it's
[01:48:46] dysfunction because it feels like it's so intrinsically connected to
[01:48:47] so intrinsically connected to masculinity
[01:48:49] masculinity >> and you feel like a failure as a man if
[01:48:50] >> and you feel like a failure as a man if you can't get a erection and pleasure
[01:48:52] you can't get a erection and pleasure your partner, right?
[01:48:53] your partner, right? >> And then for women, it was talking about
[01:48:54] >> And then for women, it was talking about um prioritizing pleasure and asking for
[01:48:57] um prioritizing pleasure and asking for more foreplay or um, you know, specific
[01:49:01] more foreplay or um, you know, specific specific stimul stimulation around the
[01:49:03] specific stimul stimulation around the clitoris or something like that. um
[01:49:05] clitoris or something like that. um because they don't want to be labeled as
[01:49:06] because they don't want to be labeled as highmaintenance or too much hard work,
[01:49:09] highmaintenance or too much hard work, etc.
[01:49:10] etc. >> And it's interesting that on that walk
[01:49:11] >> And it's interesting that on that walk with your partner, there are different
[01:49:13] with your partner, there are different things that will trigger each of you.
[01:49:15] things that will trigger each of you. >> Mhm.
[01:49:16] >> Mhm. >> Um other ones for women were pain and
[01:49:18] >> Um other ones for women were pain and discomfort, bringing that up. Um body
[01:49:20] discomfort, bringing that up. Um body image and the mental load, explaining
[01:49:22] image and the mental load, explaining why you aren't in the mood. And other
[01:49:24] why you aren't in the mood. And other ones for men are expressing nonsexual
[01:49:26] ones for men are expressing nonsexual needs because it might be seen as
[01:49:26] needs because it might be seen as unmanly, unmanly, boundaries and
[01:49:29] unmanly, unmanly, boundaries and rejection, communicating when they don't
[01:49:30] rejection, communicating when they don't want sex. And lastly, the script, which
[01:49:32] want sex. And lastly, the script, which is discussing fantasies that deviate
[01:49:33] is discussing fantasies that deviate from the norm.
[01:49:35] from the norm. >> So, I want to talk about fantasy, but
[01:49:36] >> So, I want to talk about fantasy, but before that, I would just say that, you
[01:49:38] before that, I would just say that, you know, it because of social media, we're
[01:49:40] know, it because of social media, we're in a place and I get text messages from
[01:49:43] in a place and I get text messages from my husband 24 hours a day with like not
[01:49:44] my husband 24 hours a day with like not 24 hours a day, but like all waking
[01:49:46] 24 hours a day, but like all waking hours with reals and videos. Oh, I think
[01:49:49] hours with reals and videos. Oh, I think you'd like this. I think you'd like
[01:49:50] you'd like this. I think you'd like this. You should check this out. And
[01:49:52] this. You should check this out. And it's like his way of like connecting and
[01:49:54] it's like his way of like connecting and like, hey, let's talk about this later.
[01:49:55] like, hey, let's talk about this later. Like, this is fun. And so I I tell my
[01:49:57] Like, this is fun. And so I I tell my patients like send your partner a a
[01:49:59] patients like send your partner a a video that you saw on social media that
[01:50:01] video that you saw on social media that might help them understand like why this
[01:50:03] might help them understand like why this is important to you. Maybe send them a
[01:50:05] is important to you. Maybe send them a video send them this podcast, right? Say
[01:50:07] video send them this podcast, right? Say like, "Hey, I watched this. It was
[01:50:08] like, "Hey, I watched this. It was great." Good idea. You know, um and I I
[01:50:10] great." Good idea. You know, um and I I learned so much. And honestly, so many
[01:50:12] learned so much. And honestly, so many patients bring their partners to me.
[01:50:14] patients bring their partners to me. Whether it's women bringing male
[01:50:15] Whether it's women bringing male partners or men bringing female
[01:50:17] partners or men bringing female partners, they come to me and they say,
[01:50:19] partners, they come to me and they say, "Look, like I want you to meet this
[01:50:20] "Look, like I want you to meet this doctor and I want you to talk to them."
[01:50:21] doctor and I want you to talk to them." And like it's beautiful to see that.
[01:50:24] And like it's beautiful to see that. Like I love it because it's it's so
[01:50:26] Like I love it because it's it's so deeply caring that they like brought
[01:50:28] deeply caring that they like brought their partner and they want them to feel
[01:50:30] their partner and they want them to feel good and they just want to figure out
[01:50:31] good and they just want to figure out what's going on and like I think it's so
[01:50:33] what's going on and like I think it's so lovely to to to find someone who's so
[01:50:35] lovely to to to find someone who's so invested in making sure that sex is good
[01:50:37] invested in making sure that sex is good for you, right?
[01:50:38] for you, right? >> As men though, we're just not good at
[01:50:39] >> As men though, we're just not good at talking about this stuff,
[01:50:41] talking about this stuff, >> right? But you know, I think you can't
[01:50:42] >> right? But you know, I think you can't use that as a crutch.
[01:50:44] use that as a crutch. >> Yeah.
[01:50:44] >> Yeah. >> Do you know what I mean? Like
[01:50:45] >> Do you know what I mean? Like >> Yeah, I do. Yeah. We're just not good at

[01:50:51] Women are not good at it either. >> I think men are worse at it. I think in
[01:50:53] >> I think men are worse at it. I think in general no one is good at talking about
[01:50:54] general no one is good at talking about sex. Like I, you know, I think it's
[01:50:57] sex. Like I, you know, I think it's interesting because I talk about it all
[01:50:58] interesting because I talk about it all the time, but I have seen very good
[01:51:01] the time, but I have seen very good communicators struggle with talking
[01:51:03] communicators struggle with talking about sex. And so I think it's just um
[01:51:06] about sex. And so I think it's just um it's just it's uncomfortable and and
[01:51:08] it's just it's uncomfortable and and it's like if you and I can have this
[01:51:10] it's like if you and I can have this conversation and we can talk about sex
[01:51:12] conversation and we can talk about sex openly, like there's no reason that you
[01:51:14] openly, like there's no reason that you can't learn how to talk about sex and it
[01:51:16] can't learn how to talk about sex and it it's it's a learning process and it's
[01:51:18] it's it's a learning process and it's worth it's worth investing in. Do people
[01:51:20] worth it's worth investing in. Do people bring you their fantasies?
[01:51:22] bring you their fantasies? >> Yeah, I've had I've had well more not
[01:51:24] >> Yeah, I've had I've had well more not not as many fantasies as like real sort
[01:51:27] not as many fantasies as like real sort of diverse interests in sex.
[01:51:30] of diverse interests in sex. >> What do you mean by So I had a patient
[01:51:32] >> What do you mean by So I had a patient who came to me and said uh you know
[01:51:34] who came to me and said uh you know after sometimes this was not like our
[01:51:36] after sometimes this was not like our first visit like we built a rapport and
[01:51:38] first visit like we built a rapport and he told me you know the I I really
[01:51:40] he told me you know the I I really struggle with arousal and the only time
[01:51:42] struggle with arousal and the only time I get aroused is I have a a medical kink
[01:51:45] I get aroused is I have a a medical kink and so I go to this person who will put
[01:51:47] and so I go to this person who will put a fully catheter in me and that's what
[01:51:50] a fully catheter in me and that's what turns me on. And I was like that's
[01:51:52] turns me on. And I was like that's really interesting. And I was like,
[01:51:53] really interesting. And I was like, "Okay, like that's that's great, but
[01:51:55] "Okay, like that's that's great, but like it took a long time for us to get
[01:51:56] like it took a long time for us to get there for him to feel comfortable." But
[01:51:58] there for him to feel comfortable." But it also then was like, "Well, if that's
[01:51:59] it also then was like, "Well, if that's what turns you on, you know, then you
[01:52:02] what turns you on, you know, then you got to sort of explore other ways to
[01:52:03] got to sort of explore other ways to incorporate that with a partner. Maybe
[01:52:05] incorporate that with a partner. Maybe not." So like a a catheter is is um is a
[01:52:09] not." So like a a catheter is is um is a tube that you put in the bladder for to
[01:52:11] tube that you put in the bladder for to drain urine.
[01:52:13] drain urine. >> What hole does that go in?
[01:52:14] >> What hole does that go in? >> The urethra.
[01:52:15] >> The urethra. >> It goes in the penis.
[01:52:16] >> It goes in the penis. >> Correct. And so there's actually a lot
[01:52:18] >> Correct. And so there's actually a lot of people who enjoy urethral play. So
[01:52:20] of people who enjoy urethral play. So they will take sounds, they're called
[01:52:22] they will take sounds, they're called sounds, like little um uh rods with a
[01:52:25] sounds, like little um uh rods with a frail flared base and they will insert
[01:52:27] frail flared base and they will insert them in the urethra.
[01:52:29] them in the urethra. But people do find that pleasurable. And
[01:52:31] But people do find that pleasurable. And so because there is some, you know,
[01:52:33] so because there is some, you know, there there are some nerve endings there
[01:52:34] there there are some nerve endings there that can be pleasurable. And so again,
[01:52:37] that can be pleasurable. And so again, it is totally fine to have interests
[01:52:39] it is totally fine to have interests that are, you know, outside what we call
[01:52:41] that are, you know, outside what we call conventional. Um, but of course I think
[01:52:43] conventional. Um, but of course I think you need to uh if that's something that
[01:52:45] you need to uh if that's something that you're really into, you have to sort of
[01:52:46] you're really into, you have to sort of bring your partner into the into the
[01:52:48] bring your partner into the into the fold if that's what really gets you
[01:52:50] fold if that's what really gets you turned on. Now, in terms of fantasies,
[01:52:52] turned on. Now, in terms of fantasies, almost everyone fantasizes, right?
[01:52:54] almost everyone fantasizes, right? Almost everyone does. And the
[01:52:56] Almost everyone does. And the interesting thing when you look at
[01:52:57] interesting thing when you look at fantasies is there's a lot of fantasies
[01:53:00] fantasies is there's a lot of fantasies that you would think are not common but
[01:53:02] that you would think are not common but are. So, for example, um being sexually
[01:53:05] are. So, for example, um being sexually submissive is very common. So women
[01:53:08] submissive is very common. So women fantasize about it like around 60%, men
[01:53:11] fantasize about it like around 60%, men are like 20%, having this desire of sex,
[01:53:15] are like 20%, having this desire of sex, this fantasy of sexual submission. It
[01:53:16] this fantasy of sexual submission. It may not mean that they want to be
[01:53:18] may not mean that they want to be submissive in real life, but that's the
[01:53:19] submissive in real life, but that's the fantasy they're having. Um, men often
[01:53:22] fantasy they're having. Um, men often have voyerism fantasy. So like watching
[01:53:26] have voyerism fantasy. So like watching sex. Um, they also have uh sex with
[01:53:29] sex. Um, they also have uh sex with multiple partners. Um, women tend to
[01:53:32] multiple partners. Um, women tend to have a lot of like sexual um dominance
[01:53:34] have a lot of like sexual um dominance but also romance in their fantasies. So
[01:53:36] but also romance in their fantasies. So like in exotic places or um that or with
[01:53:39] like in exotic places or um that or with and a lot of times like 90% of the time
[01:53:42] and a lot of times like 90% of the time people fantasize about strangers or it
[01:53:44] people fantasize about strangers or it could be someone they know but outside
[01:53:45] could be someone they know but outside of their relationship and so that's
[01:53:46] of their relationship and so that's completely normal and nothing to be
[01:53:48] completely normal and nothing to be worried about. It doesn't mean that like
[01:53:49] worried about. It doesn't mean that like you want that person. It's just a
[01:53:50] you want that person. It's just a fantasy. It's a safe place in your head
[01:53:52] fantasy. It's a safe place in your head to think about fantasy. And I think one
[01:53:55] to think about fantasy. And I think one realizing if someone fantasizes about
[01:53:57] realizing if someone fantasizes about something it means nothing about how
[01:53:58] something it means nothing about how they feel about you. It's just where
[01:54:00] they feel about you. It's just where their brain goes to explore. I tell my
[01:54:02] their brain goes to explore. I tell my patients sometimes write down your
[01:54:03] patients sometimes write down your fantasies for yourself. Just like write
[01:54:05] fantasies for yourself. Just like write them down. Don't share that with anybody
[01:54:06] them down. Don't share that with anybody and have your partner do the same. And
[01:54:08] and have your partner do the same. And then if you guys decide you feel
[01:54:09] then if you guys decide you feel comfortable at some point to each share
[01:54:11] comfortable at some point to each share one fantasy and you can start talk and
[01:54:13] one fantasy and you can start talk and maybe one that you would want to try and
[01:54:15] maybe one that you would want to try and like it allows you to sort of explore
[01:54:17] like it allows you to sort of explore and think about different things. It
[01:54:19] and think about different things. It could be a negative outcome like you
[01:54:20] could be a negative outcome like you could just be like oh no this what
[01:54:21] could just be like oh no this what didn't do it for me but it could also be
[01:54:23] didn't do it for me but it could also be very positive. And so just like again I
[01:54:25] very positive. And so just like again I think the big thing is understanding
[01:54:26] think the big thing is understanding like it doesn't need to be that serious.
[01:54:28] like it doesn't need to be that serious. like we can experiment, we can play, we
[01:54:30] like we can experiment, we can play, we can have fun. And if you bring that back
[01:54:33] can have fun. And if you bring that back into your life, you are going to be
[01:54:34] into your life, you are going to be happier, less lonely, live longer. Like
[01:54:38] happier, less lonely, live longer. Like everyone should just be having more sex.

[01:54:59] that way, I I see it when I open my phone.

[01:56:08] and you know, it becomes on us to take care of our fathers, too. And so, as
[01:56:09] care of our fathers, too. And so, as women, we hold that role. And I think it
[01:56:12] women, we hold that role. And I think it is so valuable to understand what men go
[01:56:16] is so valuable to understand what men go through and also to be able to support
[01:56:17] through and also to be able to support them along the way. And I think it's a
[01:56:20] them along the way. And I think it's a great gift for, you know, men in your
[01:56:22] great gift for, you know, men in your life or for, you know, older sons even
[01:56:25] life or for, you know, older sons even to like help people understand their
[01:56:27] to like help people understand their bodies. And honestly, it's a huge
[01:56:28] bodies. And honestly, it's a huge motivator. I tell people like for
[01:56:30] motivator. I tell people like for nothing else, people care about sex. And
[01:56:33] nothing else, people care about sex. And I have seen patients turn around their
[01:56:35] I have seen patients turn around their lives because they want to have better
[01:56:37] lives because they want to have better sex. They'll literally improve their
[01:56:38] sex. They'll literally improve their blood sugars. They'll improve their
[01:56:41] blood sugars. They'll improve their blood pressure because they just want to
[01:56:42] blood pressure because they just want to protect their sexual health.

[01:57:51] You know, I think um I think there is like an emergence of people starting to
[01:57:53] like an emergence of people starting to look back at religious constructs and
[01:57:56] look back at religious constructs and and go to community and be a part of
[01:57:58] and go to community and be a part of things and enjoy live events again like
[01:58:01] things and enjoy live events again like we weren't doing before. I think if we
[01:58:03] we weren't doing before. I think if we prioritize that, there is still hope
[01:58:05] prioritize that, there is still hope that we can come together as a society.
[01:58:08] that we can come together as a society. And I think, you know, the pendulum
[01:58:10] And I think, you know, the pendulum always swings right in every way, like
[01:58:12] always swings right in every way, like it's going to go far right or far left
[01:58:14] it's going to go far right or far left and people are going to hate each other
[01:58:16] and people are going to hate each other and then they're going to love each
[01:58:17] and then they're going to love each other and they're going to come back.
[01:58:18] other and they're going to come back. And I think if you just look at history
[01:58:20] And I think if you just look at history time and time again, it repeats itself.
[01:58:22] time and time again, it repeats itself. And we will find unity and we will find
[01:58:26] And we will find unity and we will find togetherness. We are obviously feeling
[01:58:28] togetherness. We are obviously feeling pain from a variety of different sources
[01:58:31] pain from a variety of different sources and we may feel pain right like in terms
[01:58:33] and we may feel pain right like in terms of other things to come but ultimately I
[01:58:36] of other things to come but ultimately I I'm an optimist and I think that we'll
[01:58:38] I'm an optimist and I think that we'll come together and and we will find um
[01:58:41] come together and and we will find um happiness and joy in human connection
